地氯雷他定分别联合地塞米松磷酸钠与卡介菌多糖核酸治疗接触性过敏性皮炎 被引量:6

Efficacy of Desloratadine combined with Dexamethasone or BCG-polysaccharide nucleic acid in the treatment of contact allergic dermatitis

摘要 目的探讨地氯雷他定分别联合地塞米松磷酸钠注射液与卡介菌多糖核酸(BCG-PSN)治疗接触性过敏性皮炎患者的疗效及对血清炎症因子的影响。方法选取2017年3月~2019年2月我科收治的82例接触性过敏性皮炎患者,采用随机数字表法分为两组进行治疗,DD组(41例)采用地氯雷他定联合地塞米松磷酸钠注射液进行治疗,DB组(41例)采用地氯雷他定联合BCG-PSN进行治疗。比较两组治疗前及治疗1个月后,血清干扰素-γ(IFN-γ)、白细胞介素-4(IL-4)、总免疫球蛋白E(IgE)、可溶性Fc段高亲和力受体(sFcεRⅠ)水平变化以及治疗1个月后两组治疗效果与药物不良反应发生率。结果治疗1个月后,两组患者血清IFN-γ、sFcεRⅠ水平均较治疗前均有显著提升,且DB组高于DD组(P<0.05);两组患者血清IL-4、总IgE水平均较治疗前均有显著下降,而DB组更低于DD组(P<0.05)。DB组治疗有效率明显高于DD组(P<0.05);两组治疗期内药物不良反应发生率比较无统计学意义(P>0.05)。结论地氯雷他定联合卡介菌多糖核酸治疗接触性过敏性皮炎临床效果较联合地塞米松磷酸钠注射液治疗方案更为理想,无明显药物不良反应,可在临床推广应用。 Objective To explore the efficacy of Desloratadine combined with Dexamethasone or BCG-polysaccharide nucleic acid(BCG-PSN)in the treatment of patients with contact allergic dermatitis and effects on serum inflammatory factors.Methods Eighty-two patients with contact allergic dermatitis were selected as study subjects.The patients were divided into DD group and DB group according to the random number table method,with 41 cases in each group.DD group was given Desloratadine combined with Dexamethasone for treatment while DB group was given Desloratadine combined with BCG-PSN for treatment.Before treatment and after 1 week and 1 month of treatment,the levels of serum interferon-γ(IFN-γ),interleukin-4(IL-4),total immunoglobulin E(IgE)and soluble Fc segment high-affinity receptor(sFcεRⅠ)were compared between the two groups,and the treatment effects after 1 month of treatment and the incidence rate of adverse drug reactions within 1 month were analyzed in the two groups.Results After 1 week and 1 month of treatment,the levels of serum IFN-γand sFcεRⅠin DB group were significantly higher than those before treatment,and were significantly higher than those in DD group(P<0.05).The levels of serum IL-4 and total IgE in the two groups were significantly lower than those before treatment,and the levels in DB group were significantly lower than those in DD group(P<0.05).The treatment effects in DB group were significantly better than those in DD group,and the effective rate of treatment was significantly higher than that in DD group(P<0.05).However,there was no significant difference in the incidence rate of adverse drug reactions between the two groups within 1 month(P>0.05).Conclusion Desloratadine combined with BCG-polysaccharide nucleic acid has more ideal effects than Desloratadine combined with Dexamethasone in the treatment of contact allergic dermatitis,and it has good drug safety,and it is beneficial to the prognosis and recovery of patients.
