1331例高血压病人中医证候分布特征及其影响因素分析

Analysis of the TCM Syndromes Distribution Characteristics and Influencing Factors of 1331 Patients with Hypertension

目的探讨高血压中医证候分布特征及影响因素,为本病证候规范化研究提供参考。方法运用上海中医药大学自行研制的高血压问诊量表采集1331例高血压病人问诊信息并进行整理,并运用频数分析及Ridit分析进行统计分析。结果1331例高血压病人中,痰湿壅盛证最多,为911例(68.4%),1级高血压病人最多,为503例,5年<病程≤10年的最多,为419例,A型血的病人最多,为490例。同时发现1级高血压与肝肾阴虚证相关,3级高血压与气虚证相关;0<病程≤5年与肝火亢盛证相关,病程>20年与肾虚证相关。结论高血压分级和病程影响因素可为高血压的辨证分型提供客观依据。

Objective To explore the distribution characteristics and influencing factors of traditional Chinese medicine(TCM)syndromes of hypertension,and to provide reference for the standardized study of hypertension syndromes.Methods One thousand three hundred and thirty-one patients with hypertension were collected and collated by the Hypertension Questionnaire developed by Shanghai University of Traditional Chinese Medicine,and frequency analysis and Ridit analysis were used for statistical analysis.Results Among the1331 hypertensive patients,patients with phlegm and dampness accumulation syndrome were the most(911 cases,68.4%),patients with Grade 1 hypertension were the most(503 cases),patients with disease more greater than 5 years and less than or equal to 10 years were the most(419 cases),patients with type A blood were the most(490 cases).At the same time,it was found that grade 1 hypertension was associated with liver and kidney yin deficiency syndrome,and grade 3 hypertension was associated with qi deficiency syndrome.The course of disease(0-5 years)was associated with the liver-fire hyperactivity syndrome,and the course of disease(>20 years)was associated with the kidney deficiency syndrome.Conclusion Grade and disease duration factors can provide an objective basis for syndrome differentiation of hypertension.
