摘要
目的通过对正常健康志愿者不同强度运动后腰椎旁肌肉进行功能磁共振序列(血氧水平依赖成像、T2-mapping)成像,分析腰椎旁肌肉(L3及L4椎体上缘层面的多裂肌、最长肌、髂肋肌)运动强度和其相应的横截面积(CSA)、横向弛豫时间(T2值)及有效横向弛豫率(R2*值)的关系,探讨不同强度运动后腰椎旁肌肉磁共振功能成像的信号变化趋势和特点。方法选取120例健康成年志愿者(男女各60例),依据不同运动强度分为4组,采用Philips Achieva 3.0 T Tx磁共振扫描仪,将所得图像传至GE ADW 4.4后处理工作站进行后处理分析,运用SPSS 25.0统计学软件分析数据。结果腰椎旁各肌肉的CSA、T2值及R2*值与运动强度存在低/中度相关,而R2*值、T2值与运动强度的关系更密切。随运动强度的增加,R2*值迅速下降至一个水平后,增加运动强度,R2*值下降平缓甚至维持平台期;总T2值随运动强度的增加呈上升的趋势;总CSA随运动强度的增加呈增加的趋势,但趋势渐缓。结论不同强度运动后腰椎旁肌肉的CSA、T2值及R2*值不同,随运动强度的增加,存在一定的变化趋势,能大致评估肌肉的生理变化状态。
Objective To analyze the lumbar paravertebral muscles(multifidus muscles at the upper edge of the L3 and L4 vertebral bodies)through functional magnetic resonance sequence(blood oxygen-dependent level imaging BOLD,T2-mapping)imaging of normal healthy volunteers after exercise with different intensities.The relationship between exercise intensity and its corresponding cross-sectional area(CSA),transverse relaxation time(T2 value)and effective transverse relaxation rate(R2*value)for multi-cleft muscle,longissimus rib muscle,were evaluated and different strengths,trends and characteristics of signal changes in magnetic resonance imaging of lumbar paravertebral muscles were observed after exercise.Methods 120 healthy adult volunteers(60 male and female)were selected and divided into 4 groups according to different exercise intensity.The images were transmitted to the US GE company ADW 4.4 using PHILIPS Achieva 3.0 T Tx magnetic resonance scanner.The research findings were post-processed and analyzed using SPSS 25.0 statistical software.Results The CSA,T2 and R2*values of the lumbar paravertebral muscles were associated with low/moderate exercise intensity,while the R2*and T2 values were more closely related to exercise intensity.With the increase of exercise intensity,the R2*value rapidly drops to a level,and the exercise intensity increases.The R2*value decreases gently and even maintains the plateau;the total T2 value increases with the increase of exercise intensity;the total CSA increases with exercise intensity.There is an increasing trend,but the trend is slows down progressively.Conclusion The CSA,T2 and R2*values of lumbar paravertebral muscles after different intensity exercises are different.With the increase of exercise intensity,there is a certain change trend,which can roughly evaluate the physiological state of muscles.
