声学微泡造影技术测定动物模型中肺血量的可行性及可重复性研究 被引量:2

The Feasibility and Repeatability of Ultrasound Microbubble Angiography in the Determination of Pulmonary Blood Volume in Animal Models

摘要 目的 通过动物实验论证超声微泡造影技术测量肺血量(pulmonary blood volume,PBV)的可行性及可重复性.方法 采用"声诺维"造影剂,利用超声微泡造影技术测量动物模型肺通过时间(pulmonary transit time,PTT)或标准化的肺通过时间(heart rate-normalized pulmonary transit time,nPTT)计算PBV,根据观察者组内、组间变异性,个体观察者日内变异性以探讨其可重复性,通过在急性缺血性左心衰动物模型中的应用以评估其可行性.结果 Bland-Atlman法统计结果显示该测量方法测量结果观察者组内一致性、日内一致性和组间一致性均较好;在急性缺血性左心衰动物模型中的应用观察发现:PTT〔(2.67±0.28)s〕较心衰前PTT〔(1.75±0.07)s〕延长,nPTT(6.72±0.57)较心衰前nPTT(3.93±0.16)增加,PBV〔(168.52±21.24)mL〕较心衰前PBV〔(108.79±11.98)mL〕增多,差异均有统计学意义(P<0.05).结论 超声微泡造影技术检测PTT或nPTT计算PBV简易可行,不易产生误判,不同观察者间检测结果一致,重复性检测结果一致性较高;通过该方法测量PTT或nPTT计算并比较PBV的变化具有一定的可行性. Objective To investigate the feasibility of measuring pulmonary blood volume(PBV)by ultrasound microbubble angiography,which may provide a feasible method for further detection of PBV changes.Methods Ultrasound microbubble angiography was used to calculate the PBV by detecting pulmonary transit time(PTT)and heart rate-normalized pulmonary transit time(nPTT).To evaluate the consensus degree based on the intra-,inter-observer and within-day variation in order to determine the repeatability.The method was used for acute left ventricular dysfunction models to determine the feasibility as well.Results The Bland-Atlman plots showed good intraobserver,within-day,and inter-observer consistency of measurement results.Application in acute left ventricular dysfunction models showed that,compared with the control,left heart failure models had higher PTT,nPTT and PBV(P<0.05).Conclusion Detection of PTT/nPTT to calculate PBV by ultrasound microbubble angiography is simple and feasible,it is not easy to produce miscarriage of justice,with good intra-and inter-observer consistency of repeatability test results.The method has certain feasibility.
