益气养阴、豁痰解毒法治疗慢性阻塞性肺病多重耐药菌临床疗效观察

Clinical Observation on Treatment of Chronic Obstructive Pulmonary Disease with Multidrug-Resistant Bacteria by Yiqi Yangyin,Huotan Jiedu Method

目的观察益气养阴、豁痰解毒法治疗慢性阻塞性肺病多重耐药菌的抗菌作用及协同抗生素增敏作用。方法选取江西省中西医结合医院呼吸内科60例慢性阻塞性肺病多重耐药菌感染住院患者采用随机单盲设计方法分为西医组与中西医组各30例,两组均采用西医标准基础治疗,中西医组在西医标准基础治疗的基础上加用益气养阴、豁痰解毒法治疗。两组疗程均为7d,观察两组治疗前后临床症状改善情况及痰培养、血液炎性指标[血白细胞计数(WBC)、超敏C反应蛋白(hs-CRP)、降钙素原(PCT)]变化情况。结果治疗后,两组临床症状明显改善,中西医组总有效率优于西医组总有效率(P<0.05);两组治疗前后痰培养细菌学疗效比较,治疗后中西医组清除率明显高于西医组清除率(P<0.05);治疗后两组血白细胞、超敏C反应蛋白及降钙素原计数均较治疗前降低,但中西医组血白细胞、超敏C反应蛋白及降钙素原计数下降程度优于西医组(P<0.05)。结论慢性阻塞性肺病多重耐药菌感染应用益气养阴、豁痰解毒法协同西医基础治疗疗效更佳,值得临床推广。

Objective To observe the antibacterial effect and synergistic antibiotic sensitization of multi-drug resistant bacteria in patients with chronic obstructive pulmonary disease(COPD)treated with Yiqi Yangyin and Huotan Jiedu method.Methods 60 patients with multiple drug resistance bacterial infection of COPD were randomly divided into western medicine group and integrated Chinese-western medicine group with 30 cases in each group.Both groups were treated with western medicine standard basic treatment,and integrated Chinese-western medicine group was treated with Yiqi Yangyin,Huotan Jiedu method on the basis of western medicine standard.The treatment course of both groups was 7 days.The improvement of clinical symptoms and the changes of sputum culture,blood inflammatory indexes(WBC,hs CRP,PCT)were observed before and after treatment.Results After treatment,the clinical symptoms of both groups were significantly improved,the total effective rate of integrated Chinese-western medicine group was higher than that of western medicine group(P<0.05);before and after treatment,the sputum culture bacteriological effect of the two groups was compared,the clearance rate of integrated Chinesewestern medicine group was higher than that of western medicine group(P<0.05);the clearance rate of white blood cells and hypersensitive C in both groups were significantly higher(P<0.05).The count of reactive protein and procalcitonin was lower than that before treatment,however,the decrease of WBC,hs CRP and procalcitonin in integrated Chinese-western medicine group was better than that in western medicine group(P<0.05).
