止血生脉散联合奥曲肽治疗肝硬化上消化道出血疗效观察

Efficacy Observation on Zhixue Shengmai Powder in Combination with Octreotide on Upper Gastrointestinal Bleeding in Cirrhosis

目的观察肝硬化上消化道出血采用止血生脉散联合奥曲肽治疗的疗效。方法以本院收治的80例肝硬化上消化道出血患者为对象,按电脑数字表法随机分作对照组与试验组,每组40例,对照组行奥曲肽治疗,试验组采取止血生脉散联合奥曲肽治疗,对比两组中医症状评分、血流动力学以及病情改善情况。结果治疗后,试验组吐血色红或紫暗、脘腹胀闷、大便色黑等症状评分较对照组低,差异有统计学意义(P<0.05);治疗后,试验组门静脉内径、脾静脉内径、门静脉血流量、脾静脉血流量、分别与对照组比较均降低差异有统计学意义(P<0.05);治疗后,试验组输血量及止血、持续便血、住院的时间均较对照组短差异有统计学意义(P<0.05)。结论肝硬化上消化道出血通过止血生脉散联合奥曲肽治疗有助于改善患者症状及血流动力学指标,并且减少输血量,缩短止血、病情缓解时间。

Objective To observe the effect of Zhixue Shengmai Powder combined with octreotide on upper gastrointestinal bleeding in cirrhosis.Methods 80 patients with cirrhosis and upper gastrointestinal hemorrhage were randomly divided into two groups according to the computer digital table method,40 patients in each group.Control group was treated with octreotide.Experimental group was treated with Zhixue Shengmai Powder and octreotide,and traditional Chinese medical(TCM)symptom score,hemodynamics and disease improvement were compared between the two groups.Results After treatment,the scores of vomiting red or purple darkness abdominal distension,and stool color were lower in experimental group than in control group,and the difference was statistically significant(P<0.05);the diameter of the portal vein were lower,the inner diameter of the splenic vein the blood flow of the portal vein and the blood flow of the splenic vein of experimental group were lower than those of control group,the difference was statistically significant(P<0.05);the transfusion volume and the time of hemostasis,continuous blood in the stool,and hospitalization of experimental group were shorter than control group,and the difference was statistically significant(P<0.05).Conclusion Upper gastrointestinal hemorrhage of liver cirrhosis combined with hemostasis and octreotide can improve the symptoms and hemodynamics of patients,reduce blood transfusion and shorten the time of hemostasis and remission.
