四逆散加减联合四联疗法治疗慢性非萎缩性胃炎临床疗效及对炎症反应的影响

Clinical Curative Effect of Modified Sini Powder Combined with Quadruple Therapy on Chronic Non-atrophic Gastritis and Its Influences on Inflammatory Response

目的观察四逆散加减联合四联疗法治疗慢性非萎缩性胃炎(CNAG)临床疗效及对炎症反应的影响。方法将我中心100例CNAG患者随机分为对照组与研究组各50例,对照组采用四联疗法(奥美拉唑、枸橼酸铋钾、克拉霉素、阿莫西林)治疗,研究组采用四逆散加减联合四联疗法治疗,比较两组治疗后的临床疗效;两组腹痛、腹胀、恶心的缓解率;治疗后幽门螺旋杆菌(Hp)的清除率;随访半年内的复发率;治疗前后两组胃粘膜炎症及病理评分情况。结果研究组治疗总有效率明显高于对照组(P<0.05);腹痛、腹胀、恶心的缓解率明显高于对照组(P<0.05);经治疗后研究组Hp清除率明显高于对照组(P<0.05);随访半年内Hp复发率明显低于对照组(P<0.05);经治疗后研究组胃粘膜炎症及病理评分均明显低于对照组(P<0.05);两组治疗过程中不良反应发生率均较低,且差异不明显(P>0.05)。结论相比单用四联疗法,四逆散加减联合四联疗法治疗CNAG的临床疗效较显著,更能改善临床症状,清除Hp、减少复发、减轻胃粘膜炎症反应及病理改变,且具有较高的用药安全性。

Objective To observe the clinical curative effect of modified Sini Powder combined with quadruple therapy on chronic non-atrophic gastritis(CNAG) and its influences on inflammatory response.Methods 100 CNAG patients were randomly divided into control group and study group,50 cases in each group.Control group was treated with quadruple therapy(omeprazole, colloidal bismuth subcitrate,clarithromycin,amoxicillin),while study group was treated with modified Sini Powder combined with quadruple therapy.The clinical curative effect, remission rates of abdominal pain,bloating and nausea,clearance rate of Helicobacter pylori(Hp) after treatment,and recurrence rate within six-month follow-up in both groups were recorded;the gastric mucosal inflammation and pathological scores in both groups before and after treatment were recorded and compared.Results The total response rate of treatment in study group was significantly higher than that in control group(P<0.05),and remission rates of abdominal pain,bloating and nausea were significantly higher than those in control group(P<0.05);Hp clearance rate in study group was significantly higher than that in control group(P<0.05),while Hp recurrence rate within sixmonth follow-up was significantly lower than that in control group(P<0.05);gastric mucosal inflammation and pathological scores in study group were significantly lower than those in control group(P<0.05).Conclusion Compared with quadruple therapy alone,clinical curative effect of modified Sini Powder combined with quadruple therapy is more significant on CNAG.It can more improve clinical symptoms,clear Hp,reduce recurrence,alleviate gastric mucosal inflammation and pathological changes,with higher medication safety.
