摘要
针对紫癜性肾炎的中医药治疗,提出了从伏邪进行辨证论治的新视角。认为伏邪是紫癜性肾炎的起病关键,滞留部位主要在肾。紫癜性肾炎发病隐匿、病势缠绵等特点,均与“伏邪感病”理论相符合。借助现代研究和认识,在免疫学应答产物、感染因素、细胞因子等层面显示出与本病的发生发展具有相关性。过敏性紫癜疾病的早期应当尽早观察捕捉肾脏指标的变化、保护肾脏功能。论治本病可结合伏邪理论,确立“透表邪、消瘀斑;除湿热、祛瘀毒;益脾肾、固根元”等特色治则治法。伏邪理论为拓展紫癜性肾炎的临床诊治思路提供了参考。
In view of the treatment of purpura nephritis with traditional Chinese medicine,this paper puts forward a new perspective of latent pathogen for syndrome differentiation and treatment.It is considered that latent pathogen is the key to the onset of purpura nephritis,and the retention site is mainly in the kidney.The characteristics of purpura nephritis,such as occult onset and lingering disease,all coincide with the theory of“latent pathogen infection”.With the help of modern research,it is found that the immune response products,infection factors,cytokines and other aspects are related to the occurrence and development of the disease.Therefore,in the early stage of Henoch-Schonlein purpura disease,it is necessary to observe and capture the changes of kidney indicators and protect kidney function as soon as possible.The characteristic therapeutic principle and method of this disease can be established as“dispersing and expelling external pathogenic factors,eliminating ecchymosis;dispelling dampness and heat,removing blood stasis and toxin;invigorating spleen and kidney,strengthening body resistance”on the basis of latent pathogen theory.The theory of latent pathogen provides a reference for expanding the clinical diagnosis and treatment of purpura nephritis.
