优化诊疗流程对急性缺血性脑卒中患者生活自理能力的效果评价

Evaluation of the effect of optimizing the diagnosis and treatment process on the self-care ability of patients with acute ischemic stroke

目的探讨优化诊疗流程对急性缺血性脑卒中(CIS)患者生活自理能力的影响。方法选取我院2017年1月—6月收治的50例实行常规诊疗流程的CIS患者作为对照组,2017年7月—12月收治的50例实行优化诊疗流程的CIS患者作为观察组。比较2组急救室停留时间及治疗前、治疗2周后神经功能缺损情况[美国国立卫生研究院卒中量表(NIHSS)评分]、日常生活自理能力评估量表(Barthel指数)评分。结果观察组急救室停留时间较对照组短,差异有统计学意义(P<0.05);治疗2周后,2组NIHSS评分较治疗前下降,Barthel指数评分较治疗前上升,且观察组变化幅度更大,差异有统计学意义(P<0.05)。结论优化诊疗流程可缩短急性缺血性脑卒中患者抢救时间,改善神经功能缺损情况,提高生活自理能力,值得推广。

Objective To explore the effect of optimizing the diagnosis and treatment process on the self-care ability of patients with acute ischemic stroke(CIS).Methods A retrospective analysis of the clinical data of 50 CIS patients admitted to our hospital from January 2017-June 2017 who underwent routine diagnosis and treatment procedures was classified as a control group,and 50 patients admitted from July 2017-December 2017 were selected.The clinical data of CIS patients with optimized diagnosis and treatment procedures were classified as observation group.The length of stay in the emergency room and the neurological deficits[National Institutes of Health Stroke Scale(NIHSS)score],Self-care Ability Assessment Scale(Barthel Index)and mortality rate were compared between the two groups of patients before and 2 weeks after treatment.Results The staying time in the emergency room of the observation group was shorter than that of the control group,and the difference was statistically significant(P<0.05).After 2 weeks of treatment,the NIHSS score of the two groups decreased compared with that before treatment,and the Barthel index increased compared with that before treatment.Large,the difference is statistically significant(P<0.05);compared with the control group,the observation group has a lower mortality rate,and the difference is statistically significant(P<0.05).Conclusion The implementation of optimized diagnosis and treatment process can shorten the rescue time of patients,improve the neurological deficit,improve the self-care ability of patients with acute ischemic stroke,and reduce the mortality rate.It is worthy of promotion.
