家庭聚集性新型冠状病毒肺炎的临床特征与CT表现及核酸检测的关系

Relationship of Clinical Features,CT Findings and Nucleic Acid of Family Clustered COVID-19

目的探讨新型冠状病毒肺炎(COVID-19)家庭聚集性发病患者的临床特点与CT及核酸检测的关系。资料与方法纳入16组46例家庭聚集发病并确诊的COVID-19患者,分析其临床特点、CT表现及核酸检测结果。结果 41例家庭聚集性COVID-19患者首诊均出现临床症状、肺部磨玻璃影改变以及核酸检测阳性结果。1例有临床症状者CT呈典型COVID-19表现,但2次核酸检测均呈阴性;2例首诊核酸检测呈阳性,但无临床症状且CT表现正常;2例有临床症状但第2次CT及核酸检测结果呈阳性。结论家庭聚集性COVID-19患者的临床症状、CT表现及核酸检测结果有一定的差异,密切接触有武汉旅居史或确诊COVID-19患者的人,无论有无临床症状,其共同生活的家庭成员均需进行CT及核酸联合检测。首次检查结果为阴性时仍需追踪复查。

Purpose To investigate the relationship of clinical features,CT findings and nucleic acid of family clustered COVID-19 patients.Materials and Methods Sixteen families of 46 patients confirmed as COVID-19 were enrolled,and the relationship of clinical features,CT findings and nucleic acid was analyzed.Results Forty-one cases of family clustered COVID-19 presented clinical symptoms with ground-glass opacities on chest CT and positive nucleic acid test results.One patient with clinical symptoms showed typical COVID-19 manifestations on CT,but the two nucleic acid tests were negative;two cases showed positive nucleic acid tests but had no clinical symptoms and normal CT findings;two cases had clinical symptoms but the second time CT and nucleic acid test results were positive.Conclusion There are differences in clinical symptoms,CT findings and nucleic acid results of family clustered COVID-19 patients.The member who had a history of sojourn in Wuhan or contact with COVID-19 patients and the other close contacts in families,no matter have any symptoms or not,should check chest CT combined with nucleic acid assays.Even though the CT findings and nucleic acid assays are negative at the first diagnosis,regular follow-up may be necessary.
