HASTE和true-FISP序列对胎盘植入征象的显出及应用

Display and Application of HASTE and true-FISP Sequence for Placenta Accreta Spectrum

目的比较半傅里叶单次激励快速自旋回波(HASTE)和真实稳态进动快速成像(true-FISP)序列对胎盘植入征象的显出效能,探讨两种序列对植入征象的应用价值。资料与方法回顾性分析151例拟诊为前置胎盘或胎盘植入病例,以手术及病理结果为确诊标准,分为胎盘植入组102例和非植入组49例,分析HASTE和true-FISP序列下植入组中胎盘植入征象的显出情况及各序列中植入征象在胎盘植入组及非胎盘植入组间的差异。结果 HASTE和true-FISP序列对子宫肌层变薄或局部中断、胎盘突入子宫肌层、胎盘局限性隆凸、胎盘内T2WI低信号带、胎盘与子宫交界面多发纡曲血管影的征象显出差异有统计学意义(P<0.05);胎盘植入组与非植入组true-FISP序列中胎盘与肌层之间低信号带消失比较,差异有统计学意义(P<0.05),两组HASTE序列中该征象差异无统计学意义(P>0.05),两组HASTE和true-FISP序列中其他征象差异有统计学意义(P<0.05)。结论 HASTE和true-FISP序列均具有胎盘植入征象显出能力,各自序列中胎盘植入征象在胎盘植入及非胎盘植入组间显出略有差异。

Purpose To compare the efficacy of half-Fourier acquisition single-shot turbo spin echo(HASTE)and true fast imaging with steady-state precession(true-FISP)sequences for displaying signs of placental invasion,and to explore the value of these two sequences in the diagnosis of placental invasion.Materials and Methods 151 cases of placenta previa or suspected placenta invasion planned to be diagnosed were retrospectively enrolled in this study,according to the surgical and pathological results,all patients were divided into 102 cases of placental invasion group and 49 cases of non placental invasion group,the difference of HASTE and true-FISP sequences for displaying signs of placental invasion group was compared.Results The signs of myometrium thinning or local interruption,placenta invades myometrium,localized bulge of placental,dark intra-placental bands,abnormal tortuous vessels between placenta and myometrium showed by the two sequences had statistical significance(P<0.05).The disappearance of the dark intra-placental bands in true-FISP sequence had statistical significance between placental invasion group and non placental invasion group(P<0.05),there was no statistical difference in the HASTE sequence between the two groups(P>0.05).In addition,all the other implantation signs in the two sequences had statistically significant differences between the two groups(P<0.05).Conclusion Both HASTE and true-FISP sequences have the ability to show implantation signs.In their respective sequences,the signs of placenta accreta show slight differences between placental invasion group and non placental invasion group.
