不同PLD选择对3D-pCASL技术定量中老年人脑血流量的影响 被引量:1

摘要 目的:分析三维准连续式动脉自旋标记成像(3D-pCASL)技术应用中,不同标记后延迟时间(PLD)的选择对于评估中脑年患者脑血流量(CBF)值的影响。方法:收集2018年1月—2020年2月于脑病科收治的脑血管病患者40例,行常规序列及3D-pCASL(PLD:1525ms、2525ms)检查,测量前循环供血区CBF值。将患者按照年龄划分为中年组和老年组,比较两组患者不同PLD所测CBF值差异;依据三维时间飞跃法磁共振血管成像(3D-TOF MRA)将中重度脑动脉硬化定义为观察组,将正常和轻度动脉硬化定义为对照组,比较两组不同PLD所测CBF值差异。结果:中年组两种PLD所测CBF值间无统计学差异(P>0.05),老年组两种PLD所测CBF值间有统计学差异(P<0.05);中重度动脉硬化患者PLD为2525ms时所测CBF值高于1525ms,而正常和轻度动脉硬化患者两种PLD所测CBF值无统计学差异(P>0.05)。结论:不同年龄阶段及动脉血管硬化程度患者于不同PLD所定量的CBF值存在差异,检查过程中需依据个体化差异行合适的PLD选择。 Objective To analyze the effect of different post labeling delay time(PLD)on the evaluation of cerebral blood flow in patients with midbrain year in the application of 3D pcasl.Methods From January 2018 to February 2020,40 patients with cerebrovascular disease in the Department of encephalopathy were collected.Routine sequence and 3D pcasl(PLD:1525ms,2525ms)were performed,and CBF of anterior circulation was measured.The patients were divided into middleaged group and elderly group according to their age,and the differences of CBF values measured by different PLD were compared between the two groups;moderate and severe cerebral arteriosclerosis was defined as the observation group according to 3D-TOF MRA,and the normal and mild arteriosclerosis were defined as the control group,and the differences of CBF values measured by different PLD between the two groups were compared.Results There was no significant difference in CBF between the two PLDs in the middle-aged group(P>0.05),but there was a significant difference between the two PLDs in the elderly group(P<0.05);the CBF value of moderate and severe arteriosclerosis patients was higher than 1525ms when the PLD was 2525ms,but there was no significant difference between the two PLDs in normal and mild atherosclerosis patients(P>0.05).Conclusion The CBF values of patients with different age and degree of arteriosclerosis are different in different PLD.It is necessary to choose appropriate PLD according to individual differences in the examination process.
