高频彩色多普勒超声对甲状腺良恶性结节的诊断价值研究

Value of high frequency color Doppler ultrasound in the diagnosis of benign and malignant thyroid nodules

目的:探讨高频彩色多普勒超声对甲状腺良恶性结节的诊断价值研究。方法:回顾性分析2018年7月-2019年12月在我院诊疗的98例疑似甲状腺良恶性结节患者的临床资料,按TI-RADS分类,51例4类,31例2类和3类,13例1类,均行高频彩色多普勒超声检查,分析诊断敏感性及特异性、超声声像图表现及CDFI血流动力学参数变化。结果:以病理结果为金标准,高频彩色多普勒超声诊断的灵敏度为97.65%(83/85)、特异度为92.31%(12/13);良性结节在形态、边界、声晕、回声、钙化、血流等与恶性结节相比有明显差异(P<0.05);良性结节的PSV、RI明显低于恶性结节,而EDV明显高于恶性结节(P<0.05)。结论:高频彩色多普勒超声对甲状腺良恶性结节的诊断价值显著,对良恶性结节的诊断灵敏度及特异度高,能有效显示超声图像及血流动力学上的特征,提高临床诊断效果。

Objective To investigate the diagnostic value of high-frequency color Doppler ultrasound in benign and malignant thyroid nodules.Methods A retrospective analysis from July 2018 to December 2019 in our hospital diagnosis and treatment of 98 cases of suspected benign and malignant thyroid nodules in patients with clinical data,according to the classification of TI-RADS,51 cases 4 classes,31 cases 2 and 3,13 cases of type 1,all line of high frequency color doppler ultrasound examination,analysis of the diagnostic sensitivity and specificity,ultrasonic ultrasonographic manifestations and changes of a CDFI hemodynamic parameters.Results With pathological results as the gold standard,the sensitivity and specificity of high-frequency color Doppler ultrasound were 97.65%(83/85)and 92.31%(12/13).Compared with malignant nodules,benign nodules have significant differences in morphology,boundary,acoustic halo,echo,calcification and blood flow(P<0.05).PSV and RI in benign nodules were significantly lower than that in malignant nodules,while EDV was significantly higher than that in malignant nodules(P<0.05).Conclusion High frequency color Doppler ultrasonography has significant diagnostic value for benign and malignant thyroid nodules,high diagnostic sensitivity and specificity for benign and malignant thyroid nodules,can effectively display ultrasonic images and hemodynamic characteristics,improve the clinical diagnosis effect.
