乳腺导管增生性病变中CK5/6、P63、CK8/18、SMA、ER的表达意义

Expression of CK5/6,P63,CK8/18,SMA,ER in mammary duct hyperplasia

目的探讨乳腺导管增生性病变中CK5/6、P63、CK8/18、SMA、ER的表达水平变化,并分析其意义。方法回顾性分析2018年3月至2019年3月于本院就诊的128例乳腺导管增生性病变患者的临床资料,均为女性,其中24例普通型导管增生、32例平坦型上皮不典型增生、44例非典型性导管增生、28例导管原位癌,均先后对4组标本的CK5/6、P63、CK8/18、SMA、ER表达水平进行检测,观察分析4组患者的CK5/6、P63、CK8/18、SMA、ER的表达水平状况及意义。结果普通型导管增生组CK5/6阳性率高于平坦型上皮不典型增生组、导管上皮不典型增生组、导管原位癌组高,差异有统计学意义(P<0.05);4组CK8/18、SMA、P63阳性率比较差异无统计学意义;普通型导管增生组ER阳性率低于平坦型上皮不典型增生组、导管上皮不典型增生组、导管原位癌组,差异有统计学意义(P<0.05)。结论乳腺导管增生性病变中CK5/6、P63、CK8/18、SMA、ER的表达状况检测评估能够准确反馈乳腺导管增生性病变情况,为临床下一步治疗方案的制定提供可靠的科学依据。

Objective To investigate the expression of CK5/6,P63,CK8/18,SMA,ER in ductal hyperplasia,And analyze its significance.Methods The clinical data of 128 patients with mammary duct hyperplasia from March 2018 to March 2019 were analyzed retrospectively,All women,Among them,24 cases of common ductal hyperplasia,32 cases of flat epithelial dysplasia,44 cases of atypical ductal hyperplasia,28 cases of ductal carcinoma in situ,All four groups were tested for CK5/6,P63,CK8/18,SMA,ER expression,Observe and analyze the expression level and significance of CK5/6,P63,CK8/18,SMA,ER in 4 groups.Results The positive rate of CK5/6 in common ductal hyperplasia group was higher than that in flat epithelial dysplasia group,ductal epithelial dysplasia group,ductal carcinoma in situ group,and the difference was statistically significant(P<0.05);CK8/18,SMA,P63 positive rate was not statistically significant in 4 groups;ER positive rate of common ductal hyperplasia group was lower than that of flat epithelial dysplasia group,ductal epithelial dysplasia group,ductal carcinoma in situ group,difference was statistically significant(P<0.05).Conclusion The evaluation of the expression of CK5/6,P63,CK8/18,SMA,ER in ductal hyperplasia can accurately feedback the situation of ductal hyperplasia,to provide reliable scientific basis for the formulation of clinical treatment plan.
