摘要
迟发性脑缺血是动脉瘤性蛛网膜下腔出血后的一种并发症,是致使不良预后的重要原因。以往认为迟发性脑缺血主要由脑血管痉挛引起,然而近年来对临床和动物模型的研究支持了迟发性脑缺血发生机制的多因素学说。为更好地防治迟发性脑缺血,本课题组综述了如今正在研究的主要发生机制,包括脑血管痉挛、微循环障碍、微血栓形成、皮质扩散性抑制和炎性反应。
Delayed cerebral ischemia(DCI)is a complication of aneurysmal subarachnoid hemorrhage and an important cause of poor prognosis.In the past,it was believed that DCI was mainly caused by cerebral vasospasm.Recent studies on clinical and ani⁃mal models have supported the multi⁃factor theory of the mechanism of DCI.In order to better prevent and treat delayed cerebral ischemia,this article reviews some of the major mechanisms under investigation including cerebral vasospasm,microcirculatory dys⁃function,microthrombosis,cortical spreading depolarizations and neuroinflammation.
