清肝解郁汤加减联合切开引流术治疗浆细胞性乳腺炎的疗效观察

Efficacy of modified qinggan jieyu decoction combined with incision and drainage in treatment of plasma cell mastitis

目的:分析清肝解郁汤加减联合切开引流术治疗浆细胞性乳腺炎的疗效。方法:选择2017年2月—2018年6月收治的浆细胞性乳腺炎患者45例,采用随机数表法分为观察组(23例)和对照组(22例)。两组均给予阿莫西林分散片治疗,对照组在此基础上采用切开引流术治疗,观察组在对照组基础上加用清肝解郁汤。比较两组临床有效率、炎症因子[肿瘤坏死因子(TNF-α)、白细胞介素-6(IL-6)]水平、临床体征改善的时间及复发率。结果:观察组临床有效率高于对照组,差异有统计学意义(P<0.05);观察组治疗后的血清TNF-α和IL-6水平低于对照组,差异有统计学意义(P<0.05);观察组乳房肿痛消失时间、体温恢复正常时间短于对照组,差异有统计学意义(P<0.05);观察组疾病复发率低于对照组,差异有统计学意义(P<0.05)。结论:清肝解郁汤加减联合切开引流术治疗浆细胞性乳腺炎,可提高临床有效率,降低炎症因子水平,加速患者康复。

Objective:To investigate the efficacy of modified qinggan jieyu decoction combined with incision and drainage in treatment of plasma cell mastitis.Methods:All 45 patients with plasma cell mastitis who admitted in the hospital from February 2018 to June 2019 were divided into observation group(23 cases)and control group(22 cases)by random number table.Two groups were given amoxicillin dispersible tablets,and control group underwent incision and drainage,and observation group was treated with qinggan jieyu decoction based on control group.The clinical effective rate,inflammatory factors[tumor necrosis factor(TNF-α),interleukin-6(IL-6)]and improvement time of clinical signs were compared between two groups.Results:The clinical effective rate of observation group was lower than that of control group(P<0.05).After treatment,the levels of serum TNF-αand IL-6 in observation group were lower than those in control group(P<0.05).The disappearance time of breast swelling pain and recovery time of temperature in observation group were shorter than those in control group(P<0.05).Conclusion:Modified qinggan jieyu decoction combined with incision and drainage in treatment of plasma cell mastitis can increase clinical effective rate,decrease the levels of inflammatory factors,and accelerate patient recovery.
