老年僵硬膝患者人工膝关节置换术的疗效分析

Clinical analysis of knee arthroplasty in elderly patients with stiff knee

目的:探讨人工全膝关节置换术治疗老年僵硬膝患者的疗效。方法:选取2008年8月—2018年8月人工全膝关节置换术治疗的老年僵硬膝患者35例(40膝),观察并比较手术前后患者膝关节活动度、膝关节功能评分(HSS)以及术后并发症。结果:所有患者均获得随访,平均随访30个月。术前膝关节活动度38°,HSS评分平均35.4分;术后3个月膝关节活动度为90°,HSS评分平均为85.4分,优良率为85.7%,并发症发生率为8.6%。结论:人工全膝关节置换术治疗老年僵硬膝患者疗效可靠,正确掌握术中截骨技术和软组织平衡,重获膝关节的稳定以及术后指导患者积极的康复训练是保证手术治疗成功的关键。

Objective:To explore the effect of total knee arthroplasty in the treatment of elderly stiff knee joint.Methods:From August 2008 to August 2018,35 patients(40 knees)were treated with total knee arthroplasty for the elderly patients with stiff knees.The knee mobility,knee function score(HSS)and postoperative complications were observed and compared before and after the operation,and statistical analysis and active rehabilitation training were conducted.Results:All patients were followed up for an average of 30 months,with an average of 38 degrees of knee joint activity before operation,an average of 35.4 points of HSS score,an average of 90 degrees of knee joint activity three months after operation,an average of 85.4 points of HSS score,an excellent rate of 85.7%,and a complication rate of 8.6%.Conclusion:Total knee arthroplasty is a reliable treatment for elderly patients with stiff knee,The key to ensure the success of the operation is to correctly master the osteotomy technology and soft tissue balance during the operation,regain the stability of the knee joint and guide the patients’active rehabilitation training after the operation.
