危机资源管理情境模拟在产后出血急救培训中的应用

Application Eeffect of Crisis Resource Management Combined with Scenario Simulation in First Aid Training of Postpartum Hemorrhage

目的:探讨危机资源管理(CRM)情境模拟在产科产后出血急救培训中的应用效果。方法:以嘉兴市妇幼保健院2019年1-3月对产科及相关医务工作者进行CRM情境模拟培训为界限,比较培训前(2018年1月1日至2018年12月31日)和培训后(2019年4月1日至2020年3月31日)产妇产后出血的发生率和严重产后出血的发生率。采用自制问卷调查参与者对培训质量和自我认知情况的评价。结果:CRM情境模拟培训后我院产后出血和严重产后出血的发生率分别为2.63%、0.47%,均低于培训前,差异均有统计学意义(P<0.05)。大部分参与者对产后出血课程的价值、实用性以及真实性评价较高,并认为反馈讨论环节很重要。结论:通过CRM情境模拟培训,能提高产科及相关医务工作者对产后出血的处理反应能力、团队合作能力、沟通能力和整体临床处理能力。

[Objective]To evaluate the effect of crisis resource management(CRM)combined with scenario simulation in emergency training for postpartum hemorrhage in obstetrics.[Method]The incidence of postpartum hemorrhage and the incidence of severe postpartum hemorrhage were compared between the period before training(January 1,2018-December 31,2018)and after training(April 1,2019-March 31,2020).A self-designed questionnaires was used to investigate the course quality and self-cognition of the participants’evaluation.[Result]The incidence rates of postpartum hemorrhage and severe postpartum hemorrhage in our hospital were 2.63%and 0.47%,respectively.Compared to before the training,the differences were statistically significant(P<0.05).Most participants rated the value,practicability and authenticity of the postpartum hemorrhage course highly,and considered the feedback discussion session very important.[Conclusion]CRM combined with scenario simulation training can improve obstetrics and related medical workers’response ability,teamwork ability,communication ability and overall ability to deal with obstetrics postpartum hemorrhage.
