肩关节注射联合发散式体外冲击波治疗对冻结肩患者肌肉功能的影响

Effect of Shoulder Injection Combined with Radial Extracorporeal Shock Wave Therapy on Muscle Functional in Patients with Frozen Shoulder

目的:探讨肩关节注射联合发散式体外冲击波(RSWT)治疗对冻结肩患者肌肉力量和功能恢复的影响。方法:将66例冻结肩患者随机为2组,对照组采用玻璃酸钠+复方倍他米松肩关节注射,治疗组在此基础上联合RSWT治疗。比较分析2组的肩关节活动度、肌肉力量、关节功能及疼痛程度变化情况。结果:治疗后,治疗组内收、外展、内旋、外旋、伸展5个动作的活动度均显著改善,且明显优于对照组(P<0.05)。2组治疗后前屈、外旋、内旋的肌肉力量均显著改善,而治疗组改善幅度更为明显(P<0.05)。治疗后,2组视觉模拟评分(VAS)较治疗前明显下降,而治疗组VAS评分均显著优于对照组(P<0.05)。结论:肩关节注射联合RSWT治疗在肩关节活动度、肌肉力量、关节功能及疼痛缓解程度方面均优于单纯肩关节注射,可作为冻结肩治疗的优选方案。

[Objective]To explore the effect of shoulder injection combined with radial extracorporeal shock wave therapy on muscle strength and functional recovery in patients with frozen shoulder.[Method]66 patients with frozen shoulder were randomly divided into two groups.The control group was injected with sodium hyaluronate and compound betamethasone in the shoulder joint,and the treatment group was treated with RSWT on this basis.The range of motion,muscle strength,joint function and pain of the two groups were compared and analyzed.[Result]After treatment,the activity of adduction,abduction,internal rotation,external rotation and extension was significantly improved in treatment group,and was significantly better than that in control group(P<0.05).The muscle strength of posterior flexion,external rotation and internal rotation was significantly improved in both groups,and the improvement was more significant in treatment group(P<0.05).Compared with before treatment,VAS scores in the two groups was significantly reduced after treatment,while VAS scores in treatment group were significantly better than that in control group(P<0.05).[Conclusion]Shoulder injection combined with RSWT is superior to single shoulder injection in terms of shoulder joint range of motion,muscle strength,joint function and pain relief,can be used as the optimal treatment scheme for frozen shoulder.
