LEEP手术对子宫颈上皮内瘤变患者术后妊娠结局及分娩方式的影响

Effect of LEEP Operation on Postoperative Pregnancy Outcome and Delivery Methods in Patients with Cervical Intraepithelial Neoplasia

目的:分析LEEP手术对子宫颈上皮内瘤变(CIN)患者术后妊娠结局及分娩方式的影响。方法:选择2018年5月-2020年5月本院收治31例孕期保健既往行LEEP手术CIN孕妇作为研究组,同期选31例CIN孕期保健未行LEEP手术孕妇作为对照组,两组产妇定期孕检并给予健康指导,在本院进行分娩,对两组术后妊娠结局及分娩方式进行比较。结果:研究组孕妇不良妊娠结局率和围生儿不良妊娠结局率分别为32.25%和16.12%,与对照组(35.48%和19.35%)相比差异无统计学意义(P>0.05);研究组产钳助产25.81%及剖宫产率45.16%,高于对照组的9.68%、19.35%,而阴道产率54.84%,比对照组80.65%低,差异均有统计学意义(P<0.05)。结论:既往采用LEEP手术治疗的CIN产妇,对其术后受孕后妊娠结局无影响,但是会使产妇剖宫产率及产钳助产率提高,这可能是与LEEP手术改变了孕妇的子宫颈长度有关。

Objective:To analyze the effects of LEEP surgery on postoperative pregnancy outcomes and delivery methods in patients with cervical intraepithelial neoplasia(CIN).Method:From May 2018 to May 2020,the 31 pregnant women with CIN who underwent LEEP surgery were selected as the study group,and 31 pregnant women who did not undergo LEEP surgery during the same period were selected as the control group.The two groups of pregnant women received regular pregnancy checkups.The health guidance was given,and delivery was carried out in this hospital,then the pregnancy outcomes and delivery methods were compared between the two groups.Result:The adverse pregnancy outcome rates of pregnant women and perinatal infants in the study group were 32.25%and 16.12%,respectively,which were not statistically different from 35.48%and 19.35%in the control group(P>0.05).In the study group,the forceps assisted delivery rate was 25.81%and the cesarean section rate was 45.16%,which were higher than 9.68%and 19.35%in the control group,while the vaginal delivery rate was 54.84%,which was lower than 80.65%in the control group,the difference was statistically significant(P<0.05).Conclusion:The use of LEEP surgery to treat CIN women has no effect on the postoperative pregnancy outcome after conception,but it will increase the maternal cesarean section rate and forceps delivery rate,which may be related to the change of the pregnant woman’s cervix length by LEEP surgery.
