鼻内窥镜下彻底切除鼻腔-鼻窦内翻性乳头状瘤的近远期临床疗效研究

Endoscopic Radical Resection of Inverted Papilloma of Nasal Cavity and Sinus:aShort-term and Long-term Clinical Study

目的探讨鼻内窥镜下彻底切除鼻腔-鼻窦内翻性乳头状瘤(SINP)的远近期效果,为临床治疗提供参考依据。方法选取鼻腔-鼻窦内翻性乳头状瘤患者100例,采用随机数表法两组,各50例。对照组行传统手术治疗,观察组于鼻内窥镜下行手术治疗。比较两组治疗效果,记录患者术中出血量、手术时间、住院时间以及治疗前、治疗后VAS评分变化,记录患者并发症发生率以及术后1个月、术后6个月复发率。结果治疗后,观察组总有效率(92.00%)明显高于对照组总有效率(76.00%)(P>0.05);观察组术中出血量低于对照组(P<0.05);观察组手术时间、住院时间均分别短于对照组(P<0.05)。治疗后两组患者VAS评分均有降低,与治疗前比较(P<0.05);且观察组低于对照组(P<0.05);观察组并发症总发生率(10.00%)低于对照组(26.00%)(P<0.05);对比术后6个月疾病复发率,观察组发生率(6.00%)低于对照组(22.00%)(P<0.05)。结论鼻内窥镜下彻底切除术治疗鼻腔-鼻窦内翻乳头状瘤具有良好的效果,缩短住院时间,缓解疼痛,减少并发症发生率,降低术后复发率,是一种高效的手术方法,值得临床推广使用。

Objective To investigate the long-term and short-term effects of endoscopic resection of inverted papilloma of nasal cavity and sinus(SINP)and to provide reference for clinical treatment.Methods 100 patients with inverted papilloma of nasal cavity and paranasal sinus were randomly divided into two groups,50 cases in each group.The control group received traditional surgical treatment,while the observation group received endoscopic sinus surgery.The treatment effect was compared between the two groups.The blood volume,operation time,hospitalization time,VAS score before and after treatment were recorded.The incidence of complications and recurrence rate at 1 month and 6 months after operation were recorded.Results After treatment,the total effective rate of the observation group was 92.00%significantly higher than that of the control group 76.00%(P>0.05);the amount of bleeding in the observation group was lower than that of the control group(P<0.05);the operation time and hospitalization time of the observation group were shorter than those of the control group(P<0.05).After treatment,the VAS scores of the 2 groups were lower than those before treatment(P<0.05),and the incidence of complications in the observation group was lower than that in the control group(P<0.05);the total incidence of complications in the observation group was 10.00%lower than that in the control group(26.00%)(P<0.05);and the recurrence rate of diseases in the observation group was 6.00%lower than that in the control group 22.00%(P<0.05).Conclusion Endoscopic radical resection of inverted papilloma of nasal cavity and paranasal sinus has a good effect,shorten hospitalization time,alleviate pain,reduce the incidence of complications,and reduce the recurrence rate.It is an effective surgical method and worthy of clinical application.
