临床药师干预抗菌药物使用的效果

目的探讨临床药师干预对抗菌药物使用情况的影响。方法通过本院临床药学管理系统与医院信息系统,回顾性分析2017年1月至2017年12月本院568例病例中的抗菌药物的使用情况及存在问题,并制订针对性的临床药师干预方案,分析临床药师干预后2018年5月至2019年5月本院502例病例中抗菌药物使用的变化情况。观察干预前后药品种类选择不当、用法用量不当、药品更换频繁、超剂量用药、超适应证用药、不合理联合用药等抗菌药物使用不合理情况变化情况。同时比较干预前后抗菌药物使用品种数、抗菌药物使用时间、抗菌药物费用占治疗费用比例。结果干预后,药品种类选择不当、用法用量不当、药品更换频繁、超剂量用药、超适应证用药、不合理联合用药率均明显低于干预前,P<0.05。临床药师干预后,抗菌药物使用品种数、抗菌药物使用时间、抗菌药物费用占治疗费用比例均明显优于干预前,均有P<0.05。结论临床药师干预可有效提高抗菌药物的合理使用,对保障用药安全、减轻患者治疗费用负担具有十分重要的意义。

Objective To explore the effect of clinical pharmacist intervention on the use of antibiotics.Methods Through the clinical pharmacy management system and hospital information system of our hospital,retrospective analysis of the use and problems of antibiotics in 568 cases from January 2017 to December 2017,and the development of targeted clinical pharmacist intervention.Analyzed the changes in antibiotic use in 502 cases in our hospital from May 2018 to May 2019 after clinical pharmacist intervention.Observe the unreasonable changes in the use of antibiotics such as improper selection of drugs,improper use and dosage,frequent drug replacement,overdose,over-indication,and unreasonable combination.At the same time,the number of antibiotics used,the time of antibacterial use,and the cost of antibacterial drugs accounted for the proportion of treatment costs before and after intervention.Results After the intervention,the improper selection of drugs,improper usage and dosage,frequent drug replacement,overdose,over-indication,and irrational combination were signifi cantly lower than before intervention,P<0.05.After the intervention of clinical pharmacists,the number of antibiotics used,the time of antibacterial use,and the proportion of antibacterial drugs accounted for significantly better than the pre-intervention,all P<0.05.Conclusion Clinical pharmacist intervention can eff ectively improve the rational use of antibiotics,which is of great signifi cance for ensuring the safety of medication and reducing the burden of treatment costs for patients.
