常规心电图与动态心电图对老年冠心病患者心肌缺血和心律失常的诊断价值比较

Comparison of the diagnostic value of routine electrocardiogram and dynamic electrocardiogram for myocardial ischemia and arrhythmia in elderly patients with coronary heart disease

目的比较动态心电图和常规心电图对老年冠心病患者心肌缺血、心律失常的诊断价值。方法抽取2018年1月至2020年1月在海阳市人民医院治疗的72例老年冠心病患者,将其分为两组。常规心电图组36例,进行普通心电图检查;动态心电图组36例,进行12导联动态心电图检查。比较两组患者心肌缺血、心律失常检出情况。结果治疗后,动态心电图组患者心肌缺血检出率为77.78%,心律失常检出率为75.00%,常规心电图组检出率分别为47.22%、50.00%,差异有统计学意义(P<0.05);与常规心电图组比较,动态心电图组对室性早搏、房性早搏二三联律及成对、左右束支阻滞、房室传导阻滞、下壁及侧壁心肌缺血、ST段改变检出率更高(P<0.05)。结论动态心电图对心肌缺血、心律失常的诊断价值高于常规心电图。

Objective To compare the diagnostic value of dynamic electrocardiogram and general electrocardiogram in the diagnosis of myocardial ischemia and arrhythmia in elderly patients with coronary heart disease,and to analyze its diagnostic value.Methods Seventy-two elderly patients with coronary heart disease treated in Haiyang People’s Hospital from January 2018 to January 2020 were selected and divided into two groups,36 patients in the routine electrocardiogram group underwent general electrocardiogram examination;36 patients in the dynamic electrocardiogram group underwent 12-lead dynamic electrocardiogram examination.The detection of myocardial ischemia and arrhythmia were compared between the two groups.Results After treatment,the detection rate of myocardial ischemia in patients with dynamic electrocardiogram was 77.78%,and the detection rate of arrhythmia was 75.00%;the detection rates of conventional electrocardiogram were 47.22%and 50.00%,respectively,the differences were significant(P<0.05);compared with the routine ECG group,the detection rate of ventricular premature beat,atrioventricular block,left and right bundle branch block,atrioventricular block,inferior wall and lateral wall myocardial ischemia and ST segment change were higher in dynamic electrocardiogram group P<0.05).Conclusions The diagnostic value of dynamic electrocardiogram for myocardial ischemia and arrhythmia is higher than that of conventional electrocardiogram.
