神经内镜血肿清除术与软通道引流术治疗慢性硬膜下血肿的效果比较

Comparison of effect of neuroendoscopic hematoma removal versus soft channel drainage on patients with chronic subdural hematoma

目的比较神经内镜血肿清除术与软通道引流术治疗慢性硬膜下血肿的效果。方法抽取2017年10月至2019年12月河南省直第三人民医院收治的50例慢性硬膜下血肿患者,根据治疗方法不同分为观察组(26例)与对照组(24例),观察组给予神经内镜血肿清除术治疗,对照组给予软通道引流术治疗。比较两组患者手术前与术后1个月神经功能、生活能力、临床疗效;比较两组患者术后1 h,1、3、7、30 d血肿清除率。结果术后1个月,观察组神经功能评分低于对照组,差异有统计学意义(P<0.05);生活能力评分高于对照组,差异有统计学意义(P<0.05);总有效率(92.30%)高于对照组(50.00%),差异有统计学意义(P<0.05);术后1 h,1、3 d血肿清除率低于对照组,差异有统计学意义(P<0.01);术后7、30 d血肿清除率高于对照组,差异有统计学意义(P<0.01)。结论神经内镜血肿清除术可有效改善患者神经功能和生活能力,增强临床疗效和血肿清除率。

Objective To compare the effect of neuroendoscopic hematoma removal and soft channel drainage on patients with chronic subdural hematoma.Methods Fifty patients with chronic subdural hematoma admitted to the Third Provincial People’s Hospital of Henan from October 2017 to December 2019 were enrolled,and divided into the observation group(n=26)and the control group(n=24)according to different treatment methods.The observation group was treated with endoscopic hematoma removal,and the control group was treated with conventional soft channel drainage.The neurological function,living ability,clinical efficacy of the two groups of patients were compared before and 1 month after operation.The hematoma clearance rates at 1 hour and 1 day,3 days,7days,30 days after operation were compared.Results One month after the operation,the neurological function score of the observation group was significantly lower than that of the control group(P<0.05);the living ability score of the observation group was significantly higher than that of the control group(P<0.05);the total effective rate of the observation group(92.30%)was higher than that of the control group(50.00%),P<0.05;the hematoma clearance rate of the observation group was significantly lower than that of the control group at 1 hour,1 day and 3 days after operation(P<0.01),but the hematoma clearance rate was higher than the control group at 7 days,30 days after operation(P<0.01).Conclusions Neuroendoscopic hematoma removal can effectively improve the nerve function and living ability,and enhance the clinical efficacy and hematoma clearance rate.
