早期平均血小板体积与脓毒症患儿病情及预后的关系

The relationship between early mean platelet volume and the condition and prognosis of children with sepsis

目的:研究早期平均血小板体积(MPV)与脓毒症患儿病情及预后的关系。方法:回顾性分析272例脓毒症患儿的临床资料,根据病情将患儿分为脓毒症组、严重脓毒症组和脓毒性休克组,根据临床结局将患儿分为幸存组和死亡组,探讨早期MPV值、小儿危重病例评分(PCIS评分)及预后三者间的关系。结果:脓毒症患儿的MPV值(8.5±1.14、8.9±1.12、10.0±1.04)和病死率(7.14%、20.43%、25.37%)随病情严重程度(脓毒症、严重脓毒症、脓毒性休克)的增加而升高,PCIS评分(80.71±4.99、76.86±7.40、73.93±8.91)则相反。死亡组患儿MPV值高于幸存组(9.7±1.07 vs 8.8±1.15),PCIS评分低于幸存组(73.79±6.62 vs 78.84±4.77)。MPV值与PCIS评分呈负相关(r=-0.271,P<0.001),与病死率呈正相关(r=0.513,P<0.001),MPV值越高,PCIS评分越低,病死率越高。结论:早期MPV值与脓毒症患儿病情及预后密切相关,早期MPV值越高,病情越严重,预后越差。

Objective:To study the relationship between early mean platelet volume and the condition,and prognosis of children with sepsis.Methods:The clinical data of 272 cases with sepsis were retrospectively analyzed,they were divided into the sepsis group,the severe sepsis group,and the septic shock group according to their conditions,and divided into the survival group and the death group according to their clinical outcomes,to explore the relationship between initial MPV value,PCIS score,and prognosis.Results:The MPV value of children with sepsis(8.5±1.14,8.9±1.12,10.0±1.04)and mortality(7.14%,20.43%,25.37%)increased with the increase of severity(sepsis,severe sepsis,septic shock),while PCIS score(80.71±4.99,76.86±7.40,73.93±8.91)were on the contrary.The MPV value in the death group was higher than that in the survival group(9.7±1.07 vs.8.8±1.15),PCIS score was lower than that in the survival group(73.79±6.62 vs.78.84±4.77).The MPV value negatively correlated with the PCIS score(r=-0.271,P<0.001)and positively correlated with the mortality(r=0.513,P<0.001),the higher the MPV value,the lower the PCIS score,the higher the mortality.Conclusion:The early MPV value is closely related to the condition and prognosis of children with sepsis.The higher the early MPV value,the more serious the disease and the worse the prognosis.
