影响不孕患者夫精宫腔内人工授精效果的因素分析

Analysis of factors influencing the effect of artificial insemination of infertile patients

目的:探讨影响不孕患者夫精宫腔内人工授精(IUI)效果的相关因素。方法:选取2016年1月—2018年10月收治的183例不孕患者,收集共264个IUI周期资料,根据患者是否成功妊娠将其分为妊娠组(27例)与失败组(156例),分析影响不孕患者IUI效果的相关因素。结果:两组年龄、排卵方案、子宫内膜厚度、人绒毛膜促性腺激素(HCG)注射日子宫内膜形态、授精时机、一周期内的IUI次数以及处理后精子密度比较,差异有统计学意义(P<0.05)。经多因素Logistic回归分析发现,年龄、排卵方案、子宫内膜厚度、HCG注射日子宫内膜形态、授精时机、一周期内的IUI次数以及处理后精子密度是影响不孕患者IUI成败的独立因素(P<0.05)。结论:影响不孕患者IUI成败的因素复杂,在治疗中要综合分析患者的相关因素,制订合适的助孕方案,以提高妊娠成功率。

Objective:To explore the related factors that affect the effect of artificial insemination(IUI)on infertility patients.Methods:A total of 264 IUI cycle data from 183 patients admitted in our hospital from January 2016 to October 2018 were selected as study samples.The patients were divided into pregnancy group(27 cases)and failure group(156 cases)according to whether the pregnancy was successful,and the relevant factors affecting the IUI effect of infertile patients were analyzed.Results:There were significant differences between the two groups in age,ovulation schedule,endometrial thickness,endometrial morphology on the day of HCG injection,insemination timing,IUI times during the week,and sperm density after treatment(P<0.05).Multivariate logistic regression analysis found that age,ovulation schedule,endometrial thickness,endometrial morphology on the day of HCG injection,insemination timing,IUI times during the week,and sperm density after treatment were independent factors affecting IUI success or infertility(P<0.05).Conclusion:The factors that affect the success or failure of IUI in infertile patients are complex.In the treatment,the relevant factors of the patient must be comprehensively analyzed to develop a suitable assisted pregnancy plan to improve the pregnancy success rate.
