氢吗啡酮对小儿尿道下裂修复术后镇痛评分、镇静评分及不良反应的影响研究

The Effect of Hydromorphone on Analgesia Score,Sedation Score and Adverse Reactions after Hypospadias Repair in Children

目的:比较氢吗啡酮和舒芬太尼对小儿尿道下裂修复术后镇痛评分、镇痛评分及不良反应的影响差异。方法:选取笔者所在医院2019年1月-2020年1月收治并给予尿道下裂修复术治疗的患儿80例。随机将患儿分成氢吗啡酮组(40例)和舒芬太尼组(40例)。氢吗啡酮组给予氢吗啡酮进行术后镇痛,舒芬太尼组给予舒芬太尼进行术后镇痛。对两组患儿术后不同时点(术后6、12、24 h)镇痛评分、镇静评分及不良反应发生情况进行观察和比较。结果:氢吗啡酮组术后6、12、24 h的VAS评分和Ramsay评分均优于舒芬太尼组,差异均有统计学意义(P<0.05);两组术后不良反应发生率均为7.50%,差异无统计学意义(P>0.05)。结论:采用氢吗啡酮应用于小儿尿道下裂修复术后镇痛,患儿的镇痛和镇静效果更好,该方案具有较好的临床应用价值。

Objective:To compare the effects of Hydromorphone and Sufentanil on analgesia score,sedation score and adverse reactions after hypospadias repair in children.Method:From January 2019 to January 2020,80 children with hypospadias who were admitted to our hospital and treated with hypospadias prosthesis were selected.The children were randomly divided into the Hydromorphone group(40 cases)and the Sufentanil group(40 cases).The Hydromorphone group was given Hydromorphone for postoperative analgesia,the Sufentanil group was given Sufentanil for postoperative analgesia.The analgesic scores,sedation scores at different time points(6,12,24 h)after surgery and adverse reactions of the two groups were observed and compared.Result:The VAS and Ramsay scores in the Hydromorphone group were better than those in the Sufentanil group at 6,12,24 h after surgery,the differences were statistically significant(P<0.05).The incidence of adverse reactions in the two groups were both 7.50%,with no statistically significant difference(P>0.05).Conclusion:The analgesia and sedation effect are better in children after hypospadias repair by Hydromorphone,which has good clinical application value.
