不同途径穿刺行冠状动脉内支架植入术对老年冠心病患者PCⅠ、PCⅢ、LN、HA的影响

Effect of Different Pathways Puncture Coronary Stent Implantation on PCⅠ, PCⅢ, LN, HA in Elderly Patients with Coronary Heart Disease

目的:探讨不同途径穿刺行冠状动脉内支架植入术(CSI)对老年冠心病患者血清I型前胶原(PCⅠ)、Ⅲ型前胶原(PCⅢ)、层粘连蛋白(LN)、透明质酸(HA)的影响。方法:选取2019年1-10月于笔者所在医院接受治疗的85例老年冠心病患者为研究对象,依据随机数字表法将其分为对照组(42例)与观察组(43例)。对照组行经股动脉途径穿刺CSI,观察组行经桡动脉途径穿刺CSI。观察两组术前及术后6个月心肌纤维化指标及并发症发生情况。结果:术后6个月,两组PCⅠ、PCⅢ、LN、HA水平均较术前降低,差异有统计学意义(P<0.05);但两组组间术后6个月PCⅠ、PCⅢ、LN、HA水平比较差异无统计学意义(P>0.05)。观察组并发症发生率为6.98%,低于对照组的26.19%,差异有统计学意义(P<0.05)。结论:经桡动脉、股动脉两种穿刺途径行CSI均可改善老年冠心病患者心肌纤维化指标,其中经桡动脉穿刺术后并发症更少。

Objective:To investigate the effect of different pathways puncture coronorary stent implantation (CSI) on serum type Ⅰ procollagen (PCⅠ),type Ⅲ procollagen (PCⅢ),laminin (LN) and hyaluronic acid (HA) in elderly patients with coronary heart disease.Method:A total of 85 elderly patients with coronary heart disease who were treated in our hospital from January to October 2019 were selected as the research objects.They were divided into the control group (42 cases) and the observation group (43 cases) according to the random number table method.The control group received femoral puncture CSI,the observation group received transradial puncture CSI.Myocardial fibrosis indexes before and 6 months after operation and complications were observed in the two groups.Result:After 6 months of operation,the PCⅠ,PCⅢ,LN,HA levels of the two groups were lower than those before operation,and the differences were statistically significant (P<0.05).But there were no significant differences in PCⅠ,PCⅢ,LN and HA levels between the two groups at 6 months after operation (P>0.05).The incidence of complications in the observation group was 6.98%,which was lower than 26.19% in the control group,and the difference was statistically significant (P<0.05).Conclusion:Both transradial and femoral puncture CSI can improve myocardial fibrosis index in elderly patients with coronary heart disease,and the postoperative complications of transradial puncture are less.
