两种关节镜技术联合玻璃酸钠治疗早中期膝骨性关节炎的疗效对比

Efficacy comparison of two arthroscopic techniques combined with sodium hyaluronate in the treatment of early and mid-stage knee osteoarthritis

目的对比两种关节镜技术联合玻璃酸钠治疗早中期膝骨性关节炎的临床效果。方法 72例早中期膝骨性关节炎患者,随机分为对照组和研究组,各36例。对照组患者实施膝关节镜下单纯病灶清理术联合玻璃酸钠治疗,研究组实施膝关节镜下病灶清理加微骨折手术联合玻璃酸钠治疗。对比两组患者治疗前后膝关节功能评分、疼痛评分以及生活质量评分,手术相关指标,治疗效果。结果治疗后,两组患者膝关节功能评分、疼痛评分以及生活质量评分均优于治疗前,且研究组膝关节功能评分、疼痛评分以及生活质量评分均优于对照组,差异具有统计学意义(P<0.05)。研究组患者术后进食时间(24.86±2.58)h、下床活动时间(1.12±0.43)d以及住院时间(4.25±1.23)d均明显短于对照组的(35.26±4.05)h、(4.95±0.26)d、(6.59±2.14)d,差异具有统计学意义(P<0.05)。研究组患者治疗优良率为80.56%,高于对照组的58.33%,差异具有统计学意义(P<0.05)。结论早中期膝骨性关节炎患者临床实施膝关节镜下病灶清理加微骨折手术联合玻璃酸钠治疗的效果较佳,可以进一步缓解患者痛感,提升患者舒适度,进而实现治疗效果的提高,改善生活质量,可在临床进行较大范围推广。

Objective To observe and compare the clinical effect of two arthroscopic techniques combined with sodium hyaluronate in the treatment of early and mid-stage knee osteoarthritis. Methods A total of 72 patients with early and mid-stage knee osteoarthritis were randomly divided into control group and research group, with 36 cases in each group. The control group was treated with arthroscopic debridement combined with sodium hyaluronate, and the research group was treated with arthroscopic debridement and microfracture surgery combined with sodium hyaluronate. The knee function score, pain score, quality of life score, operation-related indexes before and after treatment and therapeutic effect were compared between the two groups. Results After treatment, the knee function score, pain score and quality of life score of the two groups were better than those before treatment, and those scores of the research group were better than the control group, and the difference was statistically significant(P<0.05). The postoperative eating time(24.86±2.58) h, off-bed activity time(1.12± 0.43) d and the hospitalization time(4.25±1.23) d of the research group were significantly shorter than(35.26±4.05) h,(4.95±0.26) d and(6.59±2.14) d of the control group, and the difference was statistically significant(P<0.05). The treatment excellent rate of the research group was 80.56%, which was higher than 58.33% of the control group, and the difference was statistically significant(P<0.05). Conclusion Arthroscopic debridement and microfracture surgery combined with sodium hyaluronate has good effect in the early and middle stage of knee osteoarthritis, which can further relieve the pain of patients, improve the comfort and the quality of life of patients. It can be widely promoted in clinical practice.
