中药活血清胰汤促进重症急性胰腺炎恢复的回顾性研究

Retrospective study on traditional Chinese medicine Huoxue Qingyi Decoction in promoting rehabilitation of severe acute pancreatitis

目的探讨中药汤剂活血清胰汤(HQD)对重症急性胰腺炎(SAP)恢复的作用。方法回顾性研究2010年6月~2014年6月本院住院的75例SAP患者。其中38例患者(对照组)接受常规治疗,37例(HQD组)在常规治疗之外接受活血清胰汤治疗。比较两组的死亡率、急性呼吸窘迫综合征(ARDS)、高淀粉酶血症持续时间、机械通气、肾功能衰竭、出血、败血症、胰腺假性囊肿、胰腺脓肿、手术率、住院时间、住院费用等。结果HQD组住院时间和住院费用明显少于对照组(P<0.05)。HQD组高淀粉酶血症和全身炎症反应综合征(SIRS)时间明显短于对照组(P<0.05)。HQD组并发症的患者比例较对照组明显降低(45.9%vs.71.1%,P<0.05),尤其是胰腺假性囊肿(10/38 vs.2/37,P<0.05)。未发现中药相关副作用。结论HQD治疗SAP是有效的,能安全和经济地促进SAP早期康复。

Objective To explore the effect of traditional Chinese medicine Huoxue QingYi Decoction(HQD)on the rehabilitation of severe acute pancreatitis(SAP).Methods A total of 75 SAP patients in our hospital between June 2010 and June 2014 were retrospective analyzed.Among them,38 patients(control group)received conventional treatment,and 37(HQD group)received Huoxue Qingyi Decoction in addition to conventional treatment.The mortality,acute respiratory distress syndrome(ARDS),duration of hyperamylaseemia,mechanical ventilation,renal failure,bleeding,sepsis,pancreatic pseudocyst,pancreatic abscess,operation rate,hospitalization time,hospitalization cost and other indicators between two groups were compared.Results The hospitalization time and hospitalization cost of HQD group were significantly less than those of control group(P<0.05).The time of hyperamylaseemia and systemic inflammatory response syndrome(SIRS)in the HQD group was significantly shorter than that in the control group(P<0.05).The proportion of complications in the HQD group was significantly lower than that in the control group(45.9%vs.71.1%,P<0.05),especially the pancreatic pseudocyst(10/38 vs.2/37,P<0.05).No traditional Chinese medicine-related side effects were found.Conclusion HQD is effective in the treatment of SAP,and it can promote the early rehabilitation of SAP safely and economically.
