超声引导小儿桡动脉穿刺置管术对小儿患者穿刺成功率及安全性的影响 被引量:8

摘要 目的探讨超声引导小儿桡动脉穿刺置管术对小儿患者穿刺成功率及安全性的影响。方法选取2017年1月~2018年12月我院收治入院的58例拟行小儿桡动脉穿刺置管术患儿为研究对象,分为观察组与对照组,每组各29例,观察组给予超声引导小儿桡动脉穿刺置管术,对照组给予触摸定位法行桡动脉穿刺置管术。观察两组穿刺时间、穿刺次数,并比较两组首次穿刺成功率、2次成功率、穿刺失败率、并发症发生率及穿刺后的平均动脉压。结果观察组穿刺时间、穿刺次数均明显低于对照组;首次穿刺成功率、2次成功率均明显高于对照组,组间比较,差异有统计学意义(P<0.05)。两组穿刺失败率、并发症发生率比较,差异无统计学意义(P>0.05);但观察组穿刺失败率及并发症总发生率较对照组有下降趋势;两组穿刺后的平均动脉压比较,差异无统计学意义(P>0.05)。结论超声引导小儿桡动脉穿刺置管术可有效缩短穿刺时间,减少穿刺次数,可有效提高首次穿刺成功率及2次穿刺成功率,且安全性高,值得临床推广普及。 Objective To explore the effect of ultrasound-guided radial artery catheterization in children on the success rate and safety of puncture in the pediatric patients.Methods From January 2017 to December 2018,a total of 58 child patients going to undergo radial artery catheterization in children who were admitted to and treated in our hospital were taken as the subjects of research and divided into the observation group and the control group,with 29 patients in each group.The observation group was given ultrasound-guided radial artery catheterization in children,while the control group was given radial artery catheterization through touch positioning method.The puncture time and puncture times of the two groups were observed,and the first puncture success rate,the second success rate,the puncture failure rate,the incidence of complications and average arterial pressure after puncture were compared between the two groups.Results The puncture time and puncture times in the observation group were both significantly lower than those in the control group;the first puncture success rate and the second success rate of the observation group were both significantly higher than those of the control group(P<0.05 in each case).When the puncture failure rate and the incidence of complications were compared between the two groups(P>0.05),but the puncture failure rate and the total incidence of complications in the observation group showed a downward trend compared with those in the control group.There was no statistical difference in mean arterial pressure between the two groups after puncture(P>0.05).Conclusion Ultrasound-guided radial artery catheterization in children can effectively shorten the puncture time,reduce the puncture times,and effectively improve the first puncture success rate and the second puncture success rate.Also with sound safety,it is worthy of clinical promotion and popularization.
