输入性新型冠状病毒肺炎的影像学表现与变化过程

摘要 目的:探讨输入性新型冠状病毒肺炎(COVID-19)的影像特征及变化过程,以提高对本病的认识。材料和方法:回顾性分析我院收治的经病毒核酸检测证实的30例输入性COVID-19患者的临床及胸部影像资料。结果:X线检查病灶多位于两肺中下野外带(70%),表现为双肺纹理增多、模糊,肺内多发斑片状或大片状密度增高影,边缘模糊。初诊胸部CT基本为早期及进展期改变,主要呈磨玻璃密度增高影及实变影(65.5%),其次为实变或结节伴周围晕征(31%),进展期患者更常见肺部实变伴内部支气管充气、扩张或扭曲(P<0.05)。影像学复查病灶具有多样性及多灶性的动态变化特点,大多数病例预后良好。结论:输入性COVID-19的影像表现有一定的特征性,结合相关病史及实验室检查,有助于本病正确诊断,尤其是薄层CT检查能识别早期征象并及时发现病情变化,具有重大的诊疗评价和疫情防控意义。 Purpose: To explore the imaging features and changing process of imported corona virus disease 2019(COVID-19),in order to improve the understanding of the disease.Materials and Methods:Clinical and chest imaging data of 30 patients with imported COVID-19 confirmed by viral nucleic acid detection were retrospectively analyzed.Results:X-ray examination showed that most of the lesions were located in the middle and lower field of the two lungs(70%), which showed that the lung markings were increased and blurred, and there were multiple patchy or large-scale high-density shadows in the lungs, with fuzzy edges. The primary CT findings were early and progressive changes, mainly ground-glass opacity and consolidation(65.5%), followed by consolidation or nodules with peripheral halo sign(31%). Consolidation with internal air bronchogram, bronchus dilation or distortion were more common found in patients with progressive CT findings(P<0.05). Imaging review lesions with diversity and multifocal of dynamic change characteristics, most of the cases had good prognosis.Conclusion: Imaging manifestations of the imported COVID-19 have certain characteristics. Combined with the relevant medical history and laboratory examination, it is helpful for the correct diagnosis of the disease, especially the thin-slice CT examination can identify the early signs and detect the disease changes in time, which has great significance in diagnosis and treatment evaluation and epidemic prevention and control.
