摘要
目的:研究人巨细胞病毒(HCMV)即刻早期1-72(IE1-72)蛋白在上皮性卵巢癌(EOC)中的表达及其临床意义,探讨HCMV IE1-72蛋白与EOC预后的关系。方法:采用免疫组化法检测青岛大学附属医院2015年1~12月收治的66例上皮性卵巢癌及30例卵巢囊腺瘤组织中HCMV IE1-72蛋白表达水平,分析其表达与肿瘤分期、新辅助化疗(NACT)和预后的关系。结果:EOC的HCMV IE1-72蛋白阳性表达率(82%)明显高于卵巢囊腺瘤(36%),差异有统计学意义(P<0.05),HCMV IE1-72蛋白阳性表达率与EOC患者年龄、初诊癌抗原125(CA_(125))水平、肿瘤组织病理类型无明显相关性(P>0.05)。Ⅲ期、Ⅳ期EOC的HCMV IE1-72蛋白广泛阳性表达率(66%)明显高于Ⅰ、Ⅱ期(25%),差异有统计学意义(P=0.03)。术前行NACT的EOC中HCMV IE1-72蛋白广泛阳性表达率(75%)明显高于未行NACT(47%),差异有统计学意义(P=0.02)。HCMV IE1-72蛋白广泛阳性表达者的总生存期明显短于局灶阳性或阴性表达者(39个月vs 41个月,P=0.03)。结论:HCMV IE1-72蛋白在EOC中高表达,HCMV感染与EOC的发生发展有一定相关性,并影响患者预后,但其确切的肿瘤调控机制仍需进一步研究。
Objective:To study the expression and clinical significance of human cytomegalovirus(HCMV)im-mediate early 1-72(IE1-72)protein in epithelial ovarian cancer(EOC),and to explore the relationship between HCMV IE1-72 protein and the prognosis of EOC.Methods:Immunohistochemistry was used to detect the expres-sion of HCMV IE1-72 in 66 cases of epithelial ovarian cancer and 30 cases of ovarian cystadenoma in the Affilia-ted Hospital of Qingdao University from January to December 2015.The relationship between the expression of HCMV IE1-72 protein and tumor stage,neoadjuvant chemotherapy(NACT)and prognosis was analyzed.Re-sults:The positive expression rate of IE1-72 protein in EOC(82%)was significantly higher than that in ovarian cystadenoma(36%).There was statistical difference(P<0.05).The positive expression rate of IE1-72 protein was not related to the age of EOC patients,the level of CA 125 at first diagnosis and the pathological type of tumor(P>0.05).The extensive positive expression rate of IE1-72 protein in EOC of stageⅢand stageⅣ(66%)was significantly higher than that of stageⅠand stageⅡ(25%)(P=0.03).The extensive positive expression rate of HCMV IE1-72 protein in EOC with NACT before operation(75%)was significantly higher than that without NACT(47%)(P=0.02).The overall survival time of patients with extensive positive expression of IE-72 protein was significantly shorter than that of patients with focal or negative expression(39 months vs 41 months,P=0.03).Conclusions:HCMV IE1-72 protein is highly expressed in EOC.HCMV infection is related to the occurrence and development of EOC and affects the prognosis of patients.But further studies are needed for the exact mecha-nism of oncomodulation.
