肌骨超声引导下注射复方倍他米松对肩袖损伤患者疗效的影响

Effects of Compound Betamethasone Injection Guided by Musculoskeletal Ultrasound on Patients with Rotator Cuff Injury

目的:探讨肌骨超声引导下注射复方倍他米松对肩袖损伤患者疗效的影响。方法:选取2017年6月至2019年6月宁德市医院收治的肩袖损伤患者150例,按照随机数余数分组法分为对照组(采用冲击波治疗)和观察组(采用冲击波治疗联合肌骨超声引导下注射复方倍他米松治疗),每组75例。比较两组患者的临床疗效、1991年改良版加利福尼亚大学洛杉矶分校(University of California,Los Angeles,UCLA)肩关节评分、肩关节活动度(前屈、后伸、外展及内收)、疼痛评分、生活质量评分和血清指标[白细胞介素(IL) 1、IL-6、肿瘤坏死因子α(TNF-α)及P物质]水平的差异。结果:观察组患者临床治疗优良率为93.33%(70/75),明显高于对照组的80.00%(60/75),差异有统计学意义(P <0.05)。治疗后,两组患者UCLA评分、生活质量评分较治疗前明显升高,疼痛评分较治疗前明显降低;且观察组患者UCLA评分、生活质量评分明显高于对照组,疼痛评分明显低于对照组,差异均有统计学意义(P <0.05)。治疗后,两组患者肩关节活动度(前屈、后伸、外展及内收)均较治疗前明显提高,且观察组患者明显高于对照组,差异均有统计学意义(P <0.05)。治疗后,两组患者IL-1、IL-6、TNF-α及P物质水平均较治疗前明显降低,且观察组患者明显低于对照组,差异均有统计学意义(P <0.05)。结论:肌骨超声引导下注射复方倍他米松有助于进一步提高肩袖损伤患者的临床疗效。

OBJECTIVE:To probe into the effects of compound betamethasone injection guided by musculoskeletal ultrasound on patients with rotator cuff injury.METHODS:Totally 150 patients with rotator cuff injury admitted into Fujian Ningde Hospital from Jun.2017 to Jun.2019 were extracted to be divided into the control group(shockwave therapy)and observation group(shockwave therapy combined with compound betamethasone injection guided by musculoskeletal ultrasound)via the random number remainder grouping method,with 75 cases in each group.The clinical efficacy,1991 improved version of the University of California,Los Angeles(UCLA)shoulder score,the range of motion of shoulder joints(forward flexion,backward extension,abduction and adduction),pain score,quality of life score and serum indicators[interleukin(IL)1,IL-6,tumor necrosis factor-α(TNF-α),substance P]of two groups were compared.RESULTS:The excellent and good rate of the observation group was 93.33%(70/75),significantly higher than that of the control group[80.00%(60/75)],the difference was statistically significant(P<0.05).After treatment,the UCLA score and quality of life score in two groups were significantly higher than those before treatment,and the pain scores were significantly lower than those before treatment;the UCLA score and quality of life score in the observation group were significantly higher than those in the control group,and the pain scores were significantly lower than those in the control group,with statistically significant differences(P<0.05).After treatment,the range of motion of shoulder joints(forward flexion,backward extension,abduction and adduction)of the two groups were significantly improved compared to before treatment,and the observation group was significantly higher than the control group,with statistically significant differences(P<0.05).After treatment,the levels of IL-1,IL-6,TNF-αand substance P in both groups were significantly lower than those before treatment,and the observation group was significantly lower than the control group,with statistically significant differences(P<0.05).CONCLUSIONS:Compound betamethasone injection guided by musculoskeletal ultrasound can further improve the clinical efficacy of patients with rotator cuff injury.
