腹部按摩联合功能锻炼操在胃癌腹腔镜手术患者中的应用

Effects of abdominal massage combined with functional exercise on patients undergoing laparoscopic surgery for gastric cancer

目的探讨腹部按摩联合功能锻炼操对胃癌腹腔镜手术患者术后胃肠功能恢复的影响。方法选取2017年8月-2018年8月我科收治的85例行胃癌腹腔镜手术患者为研究对象,采用随机数字表法将其分为观察组(n=43)及对照组(n=42),对照组术后行腹腔镜手术常规护理,观察组在对照组基础上行腹部按摩联合功能锻炼操,比较2组术后胃肠功能恢复情况、术后并发症发生率及生活质量。结果干预后,观察组术后首次排气时间、首次排便时间、首次进食时间、术后胃管留置时间及平均住院时间均短于对照组(P<0.001)。观察组术后胃肠道并发症发生率明显低于对照组(χ^2=36.502,P<0.001)。观察组躯体症状、心理状态、社会/家庭、身体机能及生活质量总评分均高于对照组(P<0.001)。结论腹部按摩联合功能锻炼操能有效促进胃癌腹腔镜手术患者术后胃肠功能恢复,降低患者手术后胃肠道并发症发生率,提高患者生活质量。

Objective To investigate the effect of abdominal massage combined with functional exercise on the recovery of postoperative gastrointestinal function of patients undergoing laparoscopic surgery for gastric cancer.Methods Totally 85 patients admitted to our hospital for gastric cancer laparoscopic surgery between August 2017 and August 2018 were divided into an observation group(n=43)and a control group(n=42)according to a random number table.Both groups were given routine nursing after the operation,while the observation group was additionally provided with abdominal massage combined with functional exercise.The postoperative gastrointestinal function recovery,postoperative complications and life quality between two groups were compared.Results The time of the first postoperative exhaust,defecation and food taking of the observation group were significantly earlier than that of the control group,and the postoperative gastric tube indwelling time and hospital stay were significantly shorter than the latter(P<0.001).The incidences of postoperative complications of the observation group were significantly lower than that of the control group(χ^2=36.502,P<0.001).The physiological,emotional,social/family,functional,and life quality scores of the observation group were significantly higher than that of the control group(P<0.001).Conclusion Abdominal massage combined with functional exercise can effectively promote the recovery of postoperative gastrointestinal function,reduce gastrointestinal adverse reactions and better the life quality of patients undergoing laparoscopic surgery for gastric cancer.
