冠心病介入治疗患者注意偏向与自我管理能力的关系:抗逆力的中介效应

The relationship between attention bias and self-management ability of patients with percutaneous coronary intervention:the mediating effect of resilience

目的探讨冠心病介入治疗患者注意偏向与自我管理能力的关系以及抗逆力在两者间的中介效应。方法采用冠心病自我管理行为量表(CSMS)、抗逆力量表(CD-RISC)以及正性负性注意偏向量表(APNI)作为测评工具,对380例PCI患者进行横断面调查。结果 PCI患者自我管理能力总分为(73.07±19.93)分,抗逆力总分为(82.32±14.32)分,正性注意偏向(API)得分为(39.74±8.18)分,负性注意偏向(ANI)得分为(32.86±7.98)分;正性注意偏向、抗逆力与自我管理能力呈正相关(P<0.05);路径分析显示,抗逆力在正性注意偏向和CSMS间起完全中介作用,正性注意偏向无法直接预测CSMS,但能通过抗逆力间接的预测CSMS。结论抗逆力在PCI患者的正性注意偏向和自我管理能力间具有完全中介效应。提示医护人员应关注冠心病介入患者的心理健康状况,引导患者以积极的态度治疗疾病,最终提高患者的自我管理能力水平。

Objective:To explore the relations between attention bias and self-management ability of patients with percutaneous coronary intervention and to test the mediating effect of resilience between above two variables.Methods:A total of 380 patients with percutaneous coronary intervention received the cross-sectional survey with Coronary artery diseases Self-Management Scale(CSMS),Connor-Davidso Resilience Scale(CD-RICS)and Attention to Positive and Negative Information Scale(APNI).Results:The patients'CSMS score was(73.07±19.93),the total score of psychological resilience was(82.32±14.32),the attention to positive information score was(39.74±8.18)and the attention to negative information score was(32.86±7.98).The attention to positive information and psychological resilience were positively correlated with self-management ability(P<0.05).Path analysis revealed that psychological resilience served as a complete mediator between the attention to positive information and self-management ability;The attention to positive information could be used to indirectly predict self-management ability by means of psychological resilience.Conclusion:Psychological resilience is complete mediated between the attention to positive information and self-management ability of patients with percutaneous coronary intervention.Clinical nursing staff should pay attention to the status of mental health,guide patients to treat diseases with a positive attitude,and improve patients'self-management level.
