血管内超声协助介入治疗复杂慢性完全闭塞冠脉病变效果观察

The clinical curative effects of intravascular ultrasound assisted interventional therapy on complex chronic complete occlusion coronary artery lesions

目的分析血管内超声协助介入治疗复杂慢性完全闭塞冠脉病变的临床效果。方法2017年10月至2019年1月收治的72例复杂慢性完全闭塞冠脉病变患者为对象,根据随机数字表法将其分对照2组和试验组。对照组予以常规介入治疗,试验组予以血管内超声协助介入治疗,对比分析2组手术效果、手术指标、心功能、肾功能及安全性。结果试验组手术成功率94.44%,对照组手术成功率72.22%,2组比较差异有统计学意义(P<0.05);试验组X线曝光时间比对照组短,同时造影剂用量比对照组少,差异有统计学意义(P<0.05);试验组术后左心室射血分数比对照组高,6 min步行距离比对照组长,而脑钠肽水平比对照组低,差异有统计学意义(P<0.05);试验组不良事件发生率为5.56%,与对照组比较,(22.22%)更低(P<0.05)。结论血管内超声协助介入治疗复杂慢性完全闭塞冠脉病变的临床效果显著,有助于提升患者手术成功率,改善手术指标、心功能及肾功能,并且提升手术安全性。

Objective To investigate the clinical curative effects of intravascular ultrasound assisted interventional therapy on complex chronic complete occlusion coronary artery lesions.Methods A total of 72 patients with complicated chronic total occlusion coronary arterial lesions who were admitted and treated in our hospital from October 2017 to January 2019 were enrolled in the study,who were randomly divided into two groups according to the random number table method,with 36 cases in each group.The patients in control group were given routine intervention therapy,however,the patients in experimental group were given intravascular ultrasound assisted intervention therapy.The surgical effects,surgical indexes,cardiac function,renal function and safety were observed and compared between the two groups.Results The operation success rate in experimental group was 94.44%,which was significantly higher than that(72.22%)in control group(P<0.05).The X-ray exposure time and the usage amount of contrast agent in experimental group were significantly less than those in control group(P<0.05).After operation the left ventricular ejection fraction in experimental group were significantly higher than that in control group(P<0.05),and the 6-minute walking distance in experimental group was significantly longer than that in control group(P<0.05).Moreover the brain natriuretic peptide levels in experimental group were significantly lower than those in control group(P<0.05).The incidence rate of adverse events in experimental group was 5.56%,which was significantly lower than that(22.22%)in control group(P<0.05).Conclusion The intravascular ultrasound assisted intervention in treatment of complicated chronic total occlusion coronary arterial lesions has significant clinical curative effects,which is helpful to improve the success rate of surgery,improve surgical indexes,cardiac function and renal function of patients,and improve the safety of surgery.
