盐酸奥布卡因凝胶对拔牙患儿镇痛作用的评价

The analgesic effects of oxybuprocaine hydrochloride gel in children undergoing arrachement

目的评价盐酸奥布卡因凝胶对拔牙患儿的镇痛作用。方法选取2015年1月至2017年1月口腔门诊行拔牙的患儿88例年龄4~10岁,随机分为观察组(常规局部麻醉联合盐酸奥布卡因凝胶治疗)与对照组(单纯常规局部麻醉)。采用儿童畏惧调查表-牙科分量表(CFSS-DS)评价患儿在拔牙中的心理情况,采用改良儿童牙科焦虑面部表情量表(MCDASf)评价患儿在拔牙中的情绪状态。比较2组患儿拔牙操作过程中消化道症状及局部出血的不良反应发生率。结果拔牙过程中,观察组患儿CFSS-DS评分显著低于对照组(17.02±3.43和38.11±6.39,t=19.027,P<0.01)。观察组患儿MCDASf情绪评分显著低于对照组(2.28±0.39和3.56±0.59,t=11.885,P<0.001),恐惧率显著低于对照组(7.1%和52.2%,χ^2=20.932,P<0.01)。观察组患儿术后恶心呕吐的发病率与对照组(7.14%和4.35%,χ^2=0.031,P=0.860),术后局部出血观察组发生率显著低于对照组比较,差异无统计学意义(7.14%和30.8%,χ^2=8.042,P=0.005)。结论儿童行拔牙时行局部麻醉联合盐酸奥布卡因凝胶能有效减少牙科恐惧症和改善情绪,并且能降低出血的不良反应。

Objective To investigate the analgesic effects of oxybuprocaine hydrochloride gel in children undergoing arrachement.Methods A total of 88 children aged 4~10 years who underwent arrachement in our hospital from January 2015 to January 2017 were enrolled in the study,who were randomly divided into observation group and control group.The patients in observation group were treated by conventional local anesthesia combined with oxybuprocaine hydrochloride gel,however,the apteints in control group were treated by conventional local anesthesia only.The CFSS-DS scale and MCDASf scale were used to evaluate the psychological status and emotional states of patients during arrachement,moreover,theincidence rates of adverse reactions including digestive symptoms and local bleeding were observed and compared between the two groups.Results During the arrachement,the CFSS-DS scores and MCDASf scores in observation group were significantly lower than those in control group(P<0.01).Moreover the incidence rate of fear in observation group was significantly lower than that in control group(P<0.01).The incidence rate of nausea and vomit as well as the incidence rate of local bleeding after operation in observation group were lower than those in control group,but,there were no significant differences between the two groups(P>0.05).Conclusion The combination application of oxybuprocaine hydrochloride gel and conventional local anesthesia can reduce dental fear symptoms of patients,improve patient’s emotion,and decrease the incidence of local bleeding after arrachement.
