Clinical value of Cys-C and Cr detection in evaluation of renal impairment of neonates with jaundice

摘要 目的通过检测黄疸新生儿血清胱抑素C(Cys-C)、肌酐水平(Cr)水平,探讨Cys-C、Cr评估新生儿黄疸肾功能损害的临床价值。方法选取2015年8月至2016年9月期间于商洛市中心医院新生儿科收治的98例黄疸新生儿作为研究对象,检测患儿的血清Cys-C、Cr水平,评价两种指标在诊断新生儿黄疸早期肾功能损害中的灵敏度和特异度。结果98例患儿中,男52例,女46例,平均年龄(5.12±1.69)天;血清Cys-C检测阳性为50例,占总样本量的51.02%,血清Cr检测阳性为48例,占总样本量的48.98%,两种指标检测结果差异无统计学意义(McNemar检验P=0.890);血清Cys-C检测早期肾功能损害的灵敏度为52.1%,特异度为46.0%;血清Cr检测的灵敏度为46.0%,特异度为47.9%。结论血清Cr、Cys-C均是判断新生儿黄疸早期肾功能损害的较好指标,血清Cys-C灵敏度相对较高,可为临床上判断早期肾功能损害提供理论依据。 Objective To explore clinical significance of serum cystatin C(Cys-C)and creatinine(Cr)levels of neonates with jaundice in early neonatal jaundice renal injury.Methods 98 neonates with jaundice who admitted to Department of Neonatology,Shangluo Municipal Central Hospital from August 2015 to September 2016 were selected as study subjects.The serum levels of Cys-C and Cr of all enrolled neonates were detected,and sensitivity and specificity of two indexes for diagnosis of early neonatal jaundice renal injury were analysed statistically.Results There were 52 males and 46 females among 98 subjects with an average age of(5.12±1.69)days.50 neonates had positive Cys-C,accounting for 51.02%of the total,and 48 neonates had positive Cr,accounting for 48.98%of the total.There was no significant difference in positive detection rate between the two indexes(McNemar test,P=0.890).The sensitivity of serum Cr and Cys-C for diagnosis of early neonatal jaundice renal injury was 46.0%and 52.1%respectively,and the specificity of the two indexes was 47.9%and 46.0%respectively.Conclusion Serum Cr and Cys-C are both specific markers to judge early neonatal jaundice renal injury,but the sensitivity of serum Cys-C is higher than that of serum Cr.The detection of serum levels of Cys-C and Cr can provide theoretical basis for diagnosis of early renal damage.
