摘要
目的探讨乌司他丁联合Bi PAP呼吸机治疗慢性阻塞性肺气肿合并呼吸衰竭的效果及对血气指标、肺功能的影响。方法选取我院2018年1月至2019年1月收治的82例慢性阻塞性肺气肿合并呼吸衰竭患者作为研究对象,按照随机数字表法分为联合组(41例,乌司他丁+BiPAP呼吸机)和对照组(41例,Bi PAP呼吸机)。比较两组的治疗效果、血气分析指标及肺功能指标。结果联合组的治疗总有效率为90.24%,明显高于对照组的73.17%,差异具有统计学意义(P<0.05)。治疗后,两组的PaO2、pH、SpO2均升高,PaCO2均降低,且联合组明显优于对照组,差异具有统计学意义(P<0.05)。治疗后,两组的VC%、FEV1、FVC、FEV1/FVC均升高,且联合组明显高于对照组,差异具有统计学意义(P<0.05)。结论乌司他丁联合Bi PAP呼吸机治疗慢性阻塞性肺气肿合并呼吸衰竭的效果显著,能有效改善患者的血气指标,提高患者肺功能,值得临床推广应用。
Objective To observe the effect of ulinastatin combined with BiPAP ventilator in the treatment of chronic obstructive pulmonary emphysema complicated with respiratory failure and its influence on blood gas indexes and pulmonary function.Methods A total of 82 patients with chronic obstructive pulmonary emphysema complicated with respiratory failure admitted in our hospital from January 2018 to January 2019 were selected as the research objects,and the patients were divided into combined group(41 cases,ulinastatin+BiPAP ventilator)and control group(41 cases,BiPAP ventilator)according to the random number table.The treatment effects,blood gas analysis indexes and lung function indexes of the two groups were compared.Results The total effective rate of treatment in the combined group was 90.24%,which was significantly higher than 73.17%in the control group,and the difference was statistically significant(P<0.05).After treatment,PaO2,pH and SpO2 in the two groups increased,PaCO2 decreased,those in the combined group were significantly better than the control group,and the differences were statistically significant(P<0.05).After treatment,VC%,FEV1,FVC,FEV1/FVC in the two groups increased,those in the combined group were significantly higher than the control group,and the differences were statistically significant(P<0.05).Conclusion Ulinastatin combined with BiPAP ventilator in the treatment of chronic obstructive pulmonary emphysema complicated with respiratory failure has a significant effect,it can effectively improve the blood gas indexes and lung function of patients,which is worthy of clinical application.
