冷沉淀凝血因子联合血小板输注对老年开腹手术患者凝血及安全性的影响

Effects of cryoprecipitated antihemophilic factor combined with platelet transfusion on coagulation and safety of elderly patients undergoing open surgery

目的探讨冷沉淀凝血因子联合血小板输注对老年开腹手术患者凝血及安全性的影响。方法回顾性分析空军军医大学西京医院2017年1月至2019年6月收治的行开腹胃大部切除术且进行输血治疗的400例老年患者的临床资料。根据不同的输血治疗将患者分为对照组与研究组,各200例。对照组给予冷沉淀凝血因子治疗,研究组在对照组基础上给予单采血小板输注治疗。比较两组患者的凝血功能指标、临床指标及并发症发生情况。结果输注后,两组的TT、PT、APTT均缩短,PLT、Fbg水平均升高,且研究组优于对照组,差异具有统计学意义(P<0.05)。研究组的术中出血量、引流量均少于对照组,止血时间及住院时间均短于对照组,差异具有统计学意义(P<0.05)。研究组的并发症总发生率低于对照组(P<0.05)。结论冷沉淀凝血因子联合血小板输注治疗老年开腹胃大部切除术患者可获得良好的效果,改善凝血指标,安全性高。

Objective To investigate the effects of cryoprecipitated antihemophilic factor combined with platelet transfusion on coagulation and safety of elderly patients undergoing open surgery.Methods The clinical data of 400 elderly patients undergoing open subtotal gastrectomy and blood transfusion in Xijing hospital,air force military medical university from January 2017 to June 2019 were retrospectively analyzed.According to different blood transfusion treatment,the patients were divided into control group and study group,with 200 cases in each group.The control group was treated with cryoprecipitated antihemophilic factor,and the study group was given platelet transfusion on the basis of the control group.The coagulation function indexes,clinical indexes and complications of the two groups were compared.Results After infusion,TT,PT and APTT of the two groups shortened,PLT and Fbg levels increased,those in the study group were better than the control group,and the differences were statistically significant(P<0.05).The intraoperative blood loss and drainage volume of the study group were less than those of the control group,the hemostatic time and hospitalization time were shorter than those of the control group,and the differences were statistically significant(P<0.05).The total incidence of complications in the study group was lower than that in the control group(P<0.05).Conclusion Cryoprecipitated antihemophilic factor combined with platelet transfusion in the treatment of elderly patients undergoing open subtotal gastrectomy can obtain good effect,and improve coagulation indicators,with high safety.
