术前呼吸功能锻炼在肺癌患者中的应用效果

Application effect of preoperative respiratory function exercise in lung cancer patients

目的探讨术前呼吸功能锻炼在肺癌患者中的应用效果。方法选取我院2019年1月至2020年1月收治的78例肺癌患者作为研究对象,按照护理方式将其分为对照组(常规护理)和观察组(常规护理+术前呼吸功能锻炼),各39例。比较两组患者的血气指标、肺功能指标、心理状态、生活质量及并发症发生情况。结果术后1 d,两组的PaO2均下降,但观察组下降幅度低于对照组(P<0.05);术后3 d,两组的PaO2均上升,且观察组高于对照组(P<0.05);术后1、3 d,两组的PaCO2比较,差异无统计学意义(P>0.05)。术后,两组的FVC%、FEV1%、MVV%均下降,但观察组下降幅度低于对照组(P<0.05)。护理后,两组的GQOLI-74评分均提高,SAS和SDS评分均下降,且观察组优于对照组(P<0.05)。观察组的并发症总发生率低于对照组(P<0.05)。结论术前呼吸功能锻炼护理模式应用于肺癌患者中,能有效改善其肺功能指标,降低术后并发症发生率,提高患者的生活质量,值得临床推广和应用。

Objective To explore the application effect of preoperative respiratory function exercise in lung cancer patients.Methods A total of 78 cases of lung cancer patients admitted in our hospital from January 2019 to January 2020 were selected as the research objects,and the patients were divided into control group(routine nursing)and observation group(routine nursing+preoperative respiratory function exercise)by the nursing method,with 39 cases in each group.Blood gas index,lung function index,mental state,quality of life and complications were compared between the two groups.Results At 1 d after surgery,PaO2 in both groups decreased,but the decrease range in the observation group was lower than that in the control group(P<0.05);at 3 d after surgery,PaO2 in both groups increased,and that in the observation group was higher than the control group(P<0.05);there were no significant differences in PaCO2 between the two groups at 1 and 3 d after surgery(P>0.05).After surgery,FVC%,FEV1%,and MVV%in both groups decreased,but the decrease range in the observation group were lower than those in the control group(P<0.05).After nursing,GQOLI-74 scores improved in both groups,SAS and SDS scores decreased,and those in the observation group were better than the control group(P<0.05).The total incidence of complications in the observation group was lower than that in the control group(P<0.05).Conclusion The preoperative respiratory function exercise nursing mode applied in patients with lung cancer can effectively improve the lung function index,reduce the incidence of postoperative complications,and improve the quality of life of patients,which is worthy of clinical promotion and application.
