摘要
目的:探讨乌司他丁对毛细血管渗漏综合征(capillary leak syndrome,CLS)新生儿不同时间血浆血栓调节蛋白(PTM)水平及白蛋白(Alb)水平的影响。方法:选择2016年6月至2019年6月收治的100例毛细血管渗漏综合征新生儿,随机分组,对照组50例予以羟乙基淀粉治疗,观察组50例用乌司他丁治疗,比较两组治疗后不同时间PTM水平、Alb水平及药物不良反应情况。结果:治疗前及治疗后2 d,两组PTM水平比较差异无统计学意义(P>0.05),治疗后5 d、7 d观察组PTM水平低于对照组(P<0.05);治疗前及治疗后2 d,两组Alb水平比较差异无统计学意义(P>0.05),治疗后5 d、7 d观察组Alb水平高于对照组(P<0.05);观察组不良反应发生率为6.00%,对照组为2.00%,两组相比差异无统计学意义(P>0.05)。结论:乌司他丁能有效保护CLS患儿的血管内皮细胞,降低血管通透性,降低PTM水平,提高Alb水平,抑制血浆白蛋白的渗漏,且安全可靠。
Objective:To investigate the effect of ulinastatin on the plasma thromboregulatory protein(PTM)level and albumin(Alb)level of newborn with capillary leakage syndrome(CLS).Methods:a total of 100 newborns with capillary leakage syndrome admitted from June 2016 to June 2019 were randomly divided into two groups.50 patients in the control group were treated with hydroxyethyl starch,while 50 patients in the observation group were treated with ulinastatin.PTM level,Alb level and adverse drug reactions of the two groups were compared at different times after treatment.Results:there was no statistically significant difference in PTM between the two groups before and 2 days after treatment(P>0.05).PTM level of the observation group was lower than that of the control group 5 days and 7 days after treatment(P<0.05).There was no statistically significant difference in Alb level between the two groups before and 2 days after treatment(P>0.05).Alb level of the observation group was higher than that of the control group 5 days and 7 days after treatment(P<0.05).The incidence of adverse reactions was 6.000%in the observation group and 2.000%in the control group,and there was no statistically significant difference between the two groups(P>0.05).Conclusion:Ulinastatin can effectively protect the vascular endothelial cells in children with CLS,reduce the vascular permeability,reduce the LEVEL of PTM,increase the level of Alb,and inhibit the leakage of plasma albumin,which is safe and reliable.
