摘要
目的探究视网膜静脉阻塞(RVO)患者血管内皮生长因子(VEGF)及血清同型半胱氨酸(Hcy)水平检测的临床意义。方法选取2016年5月至2019年5月在该院诊治的RVO患者46例为RVO组,根据视网膜静脉阻塞类型可分为视网膜中央静脉阻塞(CRVO)组和29例视网膜静脉分支阻塞(BRVO)组27例,同期视力检查健康者52例为健康对照组。比较两组患者VEGF及血清Hcy的水平;评估VEGF、血清Hcy联合检测对RVO患者的临床诊断价值。结果RVO组患者VEGF、血清Hcy水平明显高于健康对照组,差异有统计学意义(P<0.05);CRVO组与BRVO组的VEGF、血清Hcy水平比较,差异无统计学意义(P>0.05);ROC曲线结果显示,VEGF、血清Hcy预测RVO患者的AUC为0.874、0.837,灵敏度、特异度:VEGF为80.4%、88.5%,血清Hcy为80.4%、84.6%,具有一定预测价值,且两项联合预测曲线下面积(AUC)为0.949,灵敏度、特异度为100.0%、80.8%,高于VEGF、血清Hcy单项检测,提示联合预测价值较高。结论VEGF、血清Hcy与RVO关系显著,其在RVO患者均有较高水平,对RVO患者均有一定的诊断价值,且两者联合检测的特异度、灵敏度更高,具有更高的诊断预测价值,值得在临床推广。
Objective To investigate the clinical significance of detecting the levels of vascular endothelial growth factor(VEGF)and serum homocysteine(Hcy)in patients with retinal vein occlusion(RVO).Methods Forty-six patients with RVO treated in our hospital from May 2016 to May 2019 were selected as RVO group,According to the types of retinal vein occlusion,patients were divided into CRVO group and BRVO group,with 19 and 27 cases respectively,and 52 healthy patients with visual acuity examination during the same period were selected as healthy control group.The levels of VEGF and serum Hcy were compared between the two groups and evaluated the clinical value of combined detection of VEGF and serum Hcy in patients with RVO.Results The levels of VEGF and serum Hcy in the RVO group were significantly higher than those in the control group(P<0.05).The levels of VEGF and serum Hcy in CRVO group and BRVO group were similar(P>0.05).ROC curve,according to the results of VEGF,AUC for serum Hcy predict RVO were 0.874,0.837;sensitivity,specific degrees:VEGF was 80.4%,88.5%,serum Hcy was 80.4%,84.6%,have certain predictive value,and the two combined forecasting AUC is 0.949,sensitivity,specific degrees:100.0%,80.8%,higher than that of VEGF,serum Hcy single detection,prompt combined prediction value is higher.Conclusion The correlation between VEGF and serum Hcy and RVO is significant,which has a high level in RVO patients and has certain diagnostic value for RVO patients.Moreover,the specificity and sensitivity of combined detection of the two are higher,which has a higher diagnostic predictive value and is worth promoting in clinical practice.
