针药结合对脑卒中肢体痉挛大鼠大脑皮质中GABAa受体和GAT-1表达的影响 被引量:8

Effects of combination of acupuncture and drug on the expression of GABAa receptor and GAT-1 in cerebral cortex of rats with limb spasm after stroke

摘要 目的:观察针药结合对脑卒中肢体痉挛大鼠大脑皮质中γ-氨基丁酸a型受体(GABAa受体)和γ-氨基丁酸转运蛋白(GAT-1)表达的影响,研究其作用机制。方法:45只SD大鼠分为空白组、假手术组、模型组、西药组(巴氯芬溶液)和针药结合组(电针针刺阳陵泉、曲池+加味芍药甘草汤水煎剂)共5组,每组9只,运用改良的大脑中动脉闭塞(MACO)线栓法+内囊注射N-甲基-D-天门冬氨酶(NMDA)法制作脑卒中肢体痉挛状态模型,造模成功后第1天开始治疗,连续5d。以神经功能评分和肌张力评分评价模型和治疗效果,分别运用Western blot法和RT-PCR法检测大鼠大脑皮质中GABAa受体、GAT-1的含量和GABAa受体m RNA、GAT-1 mRNA的表达。结果:治疗前,与空白组、假手术组比较,模型组大鼠神经功能和肌张力评分均显著升高(P<0.01);治疗后,与模型组比较,西药组、针药结合组评分显著降低(P<0.01)。与空白组、假手术组比较,模型组大鼠大脑皮质中GABAa受体蛋白及mRNA表达显著降低而GAT-1蛋白及mRNA显著升高(P<0.01);与模型组比较,西药组、针药结合组大鼠大脑皮质中GABAa受体蛋白及mRNA表达显著上升而GAT-1蛋白及mRNA显著降低(P<0.01);针药结合组与西药组相比无明显变化。结论:针药结合可提高大鼠GABAa受体蛋白及mRNA的表达,同时抑制GAT-1蛋白及m RNA的表达来发挥GABA的抑制性传递功能,从而达到缓解卒中后肢体痉挛的目的且疗效与西药相当。 Objective:To observe the effects of combination of acupuncture and drug on the expression ofγ-aminobutyric acid receptor a-(GABAa)andγ-aminobutyric acid transporter(GAT-1)in cerebral cortex of rats with limb spasm after stroke,and to study its mechanism.Methods:A total of 45 SD rats were randomly selected and divided into 5 groups:control group,sham operation group,model group,western medicine group(baclofen solution)and acupuncture combined with medicine group(acupuncture of Yanglingquan G 34,Quchi LI 11+Jiawei Shaoyao Gancao Decoction).Nine rats in each group were treated with modified Maco thread embolus intravesical injection of NMDA.The model of limb spasm after stroke was established on the first day after successful establishment of the model,and the treatment lasted for 5 consecutive days.The model and therapeutic effect were evaluated by nerve damage and muscular tone score.The contents of GABAa receptor,GAT-1 and the expression of GABAa receptor mRNA,GAT-1 m RNA in cerebral cortex of rats were detected by Western Blot and RT-PCR methods,respectively.Results:Before treatment,the scores of nerve function and muscle tension in the model group were significantly higher than those in the blank group and sham operation group(P<0.01).And after treatment,the scores in the western medicine group,acupuncture group and model group were significantly lower than those in the model group(P<0.01).In the model group,compared with the blank group and sham operation group,the expression of GABAa receptor and GABAa receptor mRNA in the cerebral cortex of rats was significantly decreased,while the expression of GAT-1,GAT-1 mRNA was significantly increased(P<0.01).Compared with the model group,the expression of GABAa receptor,GABAa receptor mRNA and GAT-1,GAT-1 mRNA in western medicine group and acupuncture-drug combination group were significantly higher than those in model group(P<0.01),but there was no significant change in acupuncturedrug combination group compared with western medicine group.Conclusion:The combination of acupuncture and medicine can increase the content of GABAa receptorandthe expression of GABAa receptor mRNA,and inhibit the content of GAT-1 and the expression of GAT-1 mRNA to exert the inhibitory transmission function of GABA,so as to relieve the limb spasm after stroke and the curative effect is equal to that of western medicine.
