摘要
目的研究Ⅰ型血小板结合蛋白基序的解聚蛋白样金属蛋白酶(ADAMTS1)表达水平对非小细胞肺癌(NSCLC)血管生成及预后的影响。方法收集该院2014年1—10月因NSCLC行外科手术切除的患者68例,分别取部分肺癌组织及癌旁组织进行实时荧光定量PCR检测,另取部分组织经石蜡包埋后行微血管密度(MVD)测定,统计分析各病理参数之间的相关性,并随访5年观察ADAMTS1表达水平对患者预后的影响。结果与癌旁组织相比,NSCLC原位灶中ADAMTS1 mRNA表达水平下降,并伴随血管内皮生长因子(VEGF)mRNA表达水平及MVD升高(P<0.05);ADAMTS1 mRNA的表达水平与VEGF mRNA表达水平及MVD呈负相关;ADAMTS1 mRNA的表达水平与肺癌患者病理特征无关(P>0.05),而与TNM分期显著相关(P<0.05);生存曲线显示,低表达水平ADAMTS1的NSCLC患者生存率显著下降,Cox比例风险模型显示,NSCLC患者原发灶中ADAMTS1的表达水平是影响患者预后的危险因素。结论ADAMTS1是一种NSCLC生长抑制因子,低表达水平的ADAMTS1可促进NSCLC组织中血管生成,是患者预后不良的重要影响因素。
Objective To study the effect of expression level of a disintegrin and metalloproteinase with thrombospondin motifs(ADAMTS)1 on angiogenesis and prognosis of non-small cell lung cancer(NSCLC).Methods A total of 68 patients with NSCLC who underwent surgical resection from January to October 2014 in this hospital were collected,and some lung cancer tissues and paracancerous tissues were taken for real time fluorescent quantitative PCR detection,another part of the tissue was embedded in paraffin for microvascular density(MVD)determination,statistical analysis of the correlation between various pathological parameters,and follow-up for 5 years to observe the effect of ADAMTS1 expression level on the patient′s prognosis.Results Compared with paracancerous tisse,the expression level of ADAMTS1 mRNA decreased in NSCLC in situ and accompanied by an increase in vascular endothelial growth factor(VEGF)mRNA and MVD(P<0.05).The expression level of ADAMTS1 mRNA was negatively correlated with the expression level of VEGF mRNA and MVD.The expression level of ADAMTS1 mRNA was not related to the pathological characteristics of lung cancer patients(P>0.05),but was related to The TNM stage was significantly related(P<0.05).The survival curve showed that the survival rate of NSCLC patients with low expression level of ADAMTS1 decreased significantly;Cox proportional hazard model showed the expression level of ADAMTS1 in primary lesions of NSCLC patients was a risk factor affecting the prognosis of patients.Conclusion ADAMTS1 is a NSCLC growth inhibitory factor,low expression level of ADAMTS1 may promote angiogenesis in NSCLC cancer tissues,and is an important influential factor for poor prognosis in patients.
