疫情背景下儿科住院医师规范化培训的适应性教学与管理模式探索 被引量:9

摘要 广东某三甲妇女儿童医院是儿童新型冠状病毒肺炎定点收治医院,也是广东地区最大的儿科住院医师规范化培训基地。新冠肺炎疫情时期,对儿科规培医师的带教和管理都提出了新的挑战。儿科规培医师具有独特的"两种角色",作为一名"住院医师",是儿科临床工作的基石,工作在疫情第一线;作为一名"培训学员",仍在学习过程中,尚不足以独立承担临床实践工作;本文基于管理工作实践,结合理论分析,总结了新冠肺炎疫情期间儿科规培医师在培训和管理中可能面临的风险,探索提出了疫情背景下儿科规范化培训住院医师的适应性教学模式和应急管理措施,包括根据业务能力和心理素质评估结果调整规培医师临床科室轮转方案,选择和使用适宜的线上教学工具,在线儿科教学培训的注意事项,加强新型冠状病毒肺炎防控知识的培训和考核;同时提出了疫情下儿科规培人员管理要点,包括加强人员管控,分类分期返岗,关注儿科规培医师情绪及时提供心理疏导等。这些干预措施保障疫情期间有效开展儿科规培医师教学活动,同时全面提高了儿科规培医师疫情诊治应急能力和职业防护意识,为后期的研究和其他儿科基地的管理提供参考。 Guangzhou Women and Children’s Healthcare Center,the largest base for the standard pediatric residency training,has been designated to admit those children patients infected with COVID-19. During this period,the training to and management of the pediatric residents have been facing new challenges. The pediatric residents play two unique roles: As a "resident",they are the cornerstone of pediatric clinical work,fighting the virus on the front line;and as a"trainee",they are still learners,lacking of ability to undertake the clinical work independently. By theoretical analysis,this paper summarizes the risks that may exist in the training during the pandemic based on the training management in the hospital. Furthermore,it proposes an adaptive teaching mode and emergency management measurements about the training during COVID-19,which contain the program of adjusting training residents’ rotation among the clinical departments in line with their performance and psychological quality,the choice and use of appropriate tools for online education and teaching aids for the online training,as well as the enhancement of training and testing about the prevention and control of the pandemic. Meanwhile,the key points of trainee management during the pandemic are listed,including strengthening the personnel management,returning back to clinical work in accordance with classification and time,and paying close attention to the trainee residents’ mood and giving them timely psychological consultations. All of these ensure the orderly and comprehensive implementation of teaching activities,and improve training residents’ emergency response capabilities to the diagnosis and treatment of the pandemic and professional protection awareness in order to provide some reference for the coming research and management of other pediatric residency training bases.
