隐形矫治器对成人错[牙合]畸形患者牙周指数及ABO-OGS评分的影响

The effect of invisible appliance on the periodontal index and ABO-OGS score of adult patients with malocclusion

目的探究隐形矫治器对成人错[牙合]畸形患者牙周指数及美国正畸学会客观评分系统(ABO-OGS)评分的影响。方法 选取我院2018年5月~2019年9月成人错[牙合]畸形患者74例,根据治疗方案不同分组,各37例。对照组给予固定矫治器治疗,观察组给予隐形矫治器治疗。对比两组矫治后ABO-OGS评分(牙齿排列、颊舌向倾斜度差值、邻牙接触关系、咬合关系差值、覆盖情况、邻牙边缘嵴高度差值、咬合接触、牙根平行度)、矫治前后PAR指数(上下颌颊侧区咬[牙合]关系、上下颌牙齿错位、覆[牙合]、覆盖、中线)、佩戴矫治器后1周内疼痛程度及矫治前后牙周指数[菌斑指数(PLI)、探诊深度(PD)、牙龈指数(GI)、牙龈沟出血指数(SBI)、临床附着丧失水平(CAL)]、龈沟液肿瘤坏死因子α(TNF-α)、白介素-1β(IL-1β)表达。结果 矫治后两组ABO-OGS评分(牙齿排列、颊舌向倾斜度差值、邻牙接触关系、咬合关系差值、覆盖情况、邻牙边缘嵴高度差值、咬合接触、牙根平行度)差异无统计学意义(P>0.05);矫治后两组PAR指数(上下颌颊侧区咬牙合关系、上下颌牙齿错位、覆牙合、覆盖、中线)均低于矫治前(P<0.05),但组间对比差异无统计学意义(P>0.05);佩戴矫治器后24 h、2 d观察组疼痛程度低于对照组(P<0.05);矫治后两组PLI、PD、GI、SBI、CAL均高于治疗前,但观察组低于对照组(P<0.05);矫治后两组龈沟液TNF-α、IL-1β表达均高于矫治前,但观察组低于对照组(P<0.05)。结论 隐形矫治器、固定矫治器应用于成人错牙合畸形患者,在ABO-OGS评分、PAR指数方面效果相当,但隐形矫治器可促进牙周组织健康,减轻正畸疼痛。改善牙周指数,降低ABO-OGS评分。

Objective To explore the effect of invisible appliances on the periodontal index and the American Board of Orthodontics developed the objective grading system(ABO-OGS)score in adult malocclusion patients.Methods A total of 74 adult malocclusion patients from May 2018 to September 2019 in our hospital were selected and divided into different groups according to the treatment plan,with 37 cases in each group.The control group was treated with fixed appliances,and the observation group was treated with invisible appliances.The ABO-OGS score and periodontal index[plaque index(PLI),probing depth(PD)]were compared between the two groups before and after treatment.Results The ABO-OGS scores of the observation group after correction were lower than those of the control group(P<0.05);the PLI and PD of the observation group after correction were lower than those of the control group(P<0.05).Conclusion Invisible appliance applied to adult malocclusion patients can improve periodontal index and reduce ABOOGS score.
