胃泌素-17、胃蛋白酶原及幽门螺杆菌感染率在不同胃部疾病中的表达差异 被引量:22

摘要 目的研究幽门螺杆菌(Hp),胃泌素17(G-17)和血清胃蛋白酶原(PG)检测在常见胃部病变中的表达差异。方法使用C13尿素呼气试验对156名浅表型胃炎、萎缩性胃炎、胃溃疡、胃息肉及胃癌患者进行检测HP抗体定性,并用酶联免疫法(ELISA法)检测其血清G-17和PG水平。比较HP感染率、PG和G-17水平在不同胃部疾病中的表达差异;根据是否有HP感染分为两组,比较两组间G-17和PG表达水平;将浅表型胃炎及萎缩性胃炎按照病理严重度分为轻、中、重型,分别比较不同疾病严重程度下PG和G-17水平是否存在差异。结果各组间HP感染率比较,差异有统计学意义(P=0.010);G-17和PG水平在浅表性胃炎、萎缩性胃炎、胃溃疡、胃息肉及胃癌中表达未见差异;将浅表性胃炎和萎缩性胃炎组按照病理严重程度分型后发现G-17、PGⅠ和PGⅡ水平在炎症轻、中、重度组比较,差异有统计学意义(P<0.05)。结论HP感染率在不同胃部疾病种类中具有差异性。G-17和PG水平在常见胃部疾病分型中比较,差异无统计学意义(P>0.05)。G-17、PGⅠ和PGⅡ水平在不同炎性程度胃炎中比较,差异有统计学意义(P<0.05)。HP感染、PG、G-17在胃部疾病的发生发展中发挥重要作用。 Objective To study the differences in the expression of Helicobacter pylori(HP),gastrin 17(G-17)and serum pepsinogen(PG)in common gastric diseases.Methods The C13 urea breath test was used to detect HP antibody in 156 patients with superficial gastritis,atrophic gastritis,gastric ulcer,gastric polyp and gastric cancer,and the levels of serum G-17 and PG were determined by enzyme linked immunosorbent assay(ELISA)method.The differences of HP infection rate,PG and G-17 levels in different gastric diseases were compared.Patients were divided into two groups according to the presence of HP infection,and the expression levels of G-17 and PG were compared between the two groups.Superficial gastritis and atrophic gastritis were divided into mild,moderate and severe gastritis according to the pathological severity,and the differences of PG and G-17 levels under different disease severity were compared respectively.Results There was significant difference in the HP infection rate among different groups(P=0.010).There was no difference in the expression of G-17 and PG levels in different gastric diseases(superficial gastritis,atrophic gastritis,gastric ulcer,gastric polyp and gastric cancer).And the difference was statistically significant in the levels of G-17,PGI and PGII in mild,moderate and severe inflammation groups when classifying superficial gastritis and atrophic gastritis according to pathological severity(P<0.05).Conclusion The infection rate of HP varies among different types of gastric diseases.There was no significant difference in the levels of G-17 and PG in the classification of common gastric diseases(P>0.05).The difference in the levels of G-17,PGI and PGII in gastritis with different inflammatory degrees was statistically significant(P<0.05).It is suggested that HP infection,PG and G-17 play an important role in the occurrence and development of gastric diseases.
