Application of Chronic Diseases Trajectory Model in Nursing Management

随着护理模式的不断转变,慢性病管理成为国内外学者研究的热点话题。近年来,慢性疾病轨迹理论逐渐成为慢性病管理研究的新趋势,该理论不仅被国外学者应用于多种慢性病管理中,而且在我国医疗卫生领域的应用研究地位也越发重要。从慢性疾病轨迹理论的概述、应用途径以及该理论在慢性病管理的应用效果等方面进行综述,以期为我国慢病护理管理提供应用启示。

With the continuous transformation of nursing models,chronic disease management has become a hot topic of research for scholars at home and abroad.In recent years,the chronic illness trajectory model has gradually become a new trend in chronic disease management research.This model is not only used by foreign scholars in chronic illness management,but also has an increasingly important status in the field of medical and health research in our country.In this review,the author summarized the concept of the chronic illness trajectory model,the application approach and the application effect of the model in chronic illness management for providing application enlightenment in nursing management of chronic diseases in our country.
